Determine if other Division programs are already addressing Aboriginal Health issues: • Antenatal Shared Care • Immunisation • Chronic Disease Management • Diabetes prevention and management • Lifestyle Modification Programs • Mental Health – ATAPS, Shared Care, MHNs • Aged Care Access Initiative • AGPN Rural Palliative Care Projects

  5. What are other programs doing? Embed Aboriginal Health as a key element of the Division’s practice support role

  6. CPD events and programs Include Aboriginal Health into every CPD activity across all programs: • Case studies • Guest speakers from the community • Clinical leaders from other health services • Updates from Board members at CPD events

  7. Communications • Division website • Newsletters • Mail-outs • Fax-outs • Emails • AGMs and board meetings
