No ID Cards will be issued without the appropriate signatures. A copy of this form will be given to the contractor for the Contractor company. An ID Card will only be issued for all areas provided that all shaft areas have signed as well as the responsible person for all areas. Where the contractor is staying in the Hostel, the Hostel Clerk must first sign the form before collection of the ID Cards.
