  2. Data Capture Neonatal.Net • THANK YOU!

  3. Overview Data Capture on Neonatal Unit: Neonatal.Net Storage: Clevermed Server Transfer to NDAU: NHS Connection Extraction: Clevermed Reporting: NDAU/NNAP Receive Data: NHS Server at NDAU Create Database: NDAU Database Review Data: NDAU

  4. Data Capture Neonatal.Net • Static items – recorded once (1st admission) • Name, NHS number, gestational age, birthweight • Episodic items – recorded at each episode of care • Admission and discharge time, infection, diagnoses • Daily items – recorded at each day of care • Enteral feeding today, respiratory care today

  5. Storage • Securely uploaded and held on NHS Server (Clevermed, Ltd) • Data are truly ‘raw’ NHS Server

  6. Extraction • Information for neonatal units with • Caldicott Guardian permission only • All patient identifiers removed • No names or addresses given • NHS numbers encrypted • All dates (including date of birth) converted into minutes Clevermed

  7. Transfer NHS Connection • Data encrypted • Transferred via NHS connection • Data received and held on NHS server at NDAU

  8. Review Data • Examine the data • Completeness • Gestational age for all babies • Infection form: Sample recorded as taken but no pathogen results recorded thereafter Dataset Incomplete data entry NNAP AUDIT

  9. Create NDAU Database • Assign numeric equivalents to frequently occurring free text items • Enteral feeding: (Formula name) → ‘Formula’ • Assign numeric equivalents to string items • Gender: ‘Female’ → ‘0’ / ‘Male’ → ’1’ • Remove duplicate items • If Admission Time of Episode 2 is beforeDischarge Time of Episode 1 then all days of care in Episode 1 will be duplicated in Episode 2 Inaccurate time entry NDAU Database

  10. Create NDAU Database Neonatal.Net • Static items – recorded once (1st admission) • Name, NHS number, gestational age, birthweight • Episodic items – recorded at each episode of care • Admission and discharge time, infection, diagnoses • Daily items – recorded at each day of care • Enteral feeding today, respiratory care today

  11. Create NDAU Database • Link static data items to episodic and daily items to create a dataset • Derive variables • ROP data for NNAP Episodic available in the following Neonatal.Net forms : Daily-details form, Ad-hoc ROP form, Discharge summary, Clinical summary of stay • Infection data for NNAP Episodic available in the following Neonatal.Net forms: Daily-details form, Ad-hoc Infection form, Discharge summary, Clinical summary of stay Data entered in multiple locations NNAP AUDIT ITEMS

  12. Deriving data items: ROP Clinical summary of stay Ad hoc ROP form NNAP Discharge diagnosis, Principal diagnosis Daily details

  13. Deriving ROP Ad hoc ROP form Daily details

  14. Reporting: Index of Multiple Deprivation

  15. Reporting: Breast Feeding by Network of Booking

  16. Summary • Thank you for data submission • Help improve neonatal care and baby outcomes through audit • Audit requires high quality data • Completeness • Accuracy • Raw data • Daily clinical records

  17. Acknowledgements • All neonatal units contributing to the NDAU • NDAU Team • NDAU Steering Board • Jane Abbott (BLISS) Jacquie Kemp • Prof. Peter Brocklehurst Prof. Azeem Majeed • Prof. Kate Costeloe Prof. Neena Modi • Prof. Liz Draper Prof. Andrew Wilkinson • Imperial College London Academic Neonatal Medicine Unit

  18. Reporting: Completeness of BAPM Data Items • AdmitUnitCode (BAPM-Item 1 Name of Hospital) • DeprivationScore (Derived from BAPM-Item 3 Postcode of mother’s residence at time of birth) • IntendedPlaceofDelivery (BAPM-Item 4:Planned place of delivery at booking) • PlaceofBirth (BAPM – Item 5 Place of Birth) • AnonPatientId (Derived from BAPM-Item 6 Baby’s NHS Number) • AdmitFromUnitCode (BAPM – Item 9: Source of admission to unit) • AdmissionMonth/AdmissionTime (BAPM – Item’s 10 and 11 Date and Time of Admission) • Birthweight (BAPM Item 12) • GAWeeks (BAPM Item 13: best estimate of gestational age at delivery) • Gender (BAPM Item 14) • MultipleTotal ( BAPM Item 15: Number of fetuses) • MultipleIndicator (BAPM Item 16: Birth Order) • SteroidsGiven (BAPM Item 17: Antenatal steroids) • DischargeTime (BAPM Item 18) • DischargeStatus (BAPM Item 20: Discharge or Transfer destination) • ChestDrain/Tracheostomy (BAPM Item 27: Air leak requiring drainage) • ROPSurgery (BAPM Item 29: Therapy for retinopathy of prematurity) • VPShunt (BAPM Item 32: Surgical procedures as a result of post haemorrhagic hydrocephalus) • PDASurgery (BAPM Item 33: Surgery for PDA) • NECSurgery (BAPM Item 34: Surgery for NEC) • RespiratorySupport (BAPM Item 35: Days when an endotracheal tube is in situ/ Item 36: Days on CPAP) • CareLevel (BAPM Item 38: Number of IC days/Item 39: Number of HDC days/Item 40: Number of special care days)
