  3. Student Responsibilities • Make an appointment each semester with a DSP&S counselor to update your accommodations and educational plan

  4. Accommodations Form • Your specific accommodations will be check marked on this form

  5. Accommodations Alternate Media Adaptive Equipment Assistive Technology

  7. American Sign Language Interpreting • American Sign Language (ASL) Interpreting is available for the deaf/hearing impaired • Please contact the main office for scheduling

  8. Test Proctoring Take your tests with the Test Proctor instead of in class. Students benefit from: • Extended time • Quiet and reduced stress environment • Other services as outlined by the student accommodations form

  9. Test Proctoring Instructions • Student responsibilities for receiving Test Proctoring Services

  10. Test Appointment Request Form • Tests are scheduled one week in advance. • Forms can be picked up at the Test Proctors in the High Tech Center or online at http://www.cuyamaca.edu/dsps/forms_policies.asp
