What is a DEA 222 Form? • The form is used to order Schedule 2 controlled substances from a distributor • A DEA 222 form is a triplicate form • Brown, Green and Blue forms • Supplier must retain copy 1 • Copy 2 is sent to the DEA (by supplier) • Purchaser must retain copy 3, and all unaccepted/defective copies with each statement attached • The form should not be pre-signed • Records should be kept for at least Three 3 years

  3. How Do You Get Them? • https://apps.deadiversion.usdoj.gov/webforms/orderFormsRequest.jsp • Need DEA Number of the Physician/Medical Director • Must have the Center Name

  5. Things to Keep in Mind • DEA will send over the maximum number of allowed forms, usually SEVEN • Keep the forms locked and in order • Do not have the Medical Director pre-sign forms • If the Medical Director changes, all unused DEA forms must be sent back

Power of Attorney Form- POA • A registrant may authorize one or more individuals, whether or not located at his or her registered location, to issue orders for Schedule I and II controlled substances on the registrant's behalf by executing a power of attorney for each such individual, if the power of attorney is retained in the files, with executed Forms 222 where applicable, for the same period as any order bearing the signature of the attorney. • The power of attorney must be available for inspection together with other order records.

  11. Made a mistake? • Do not rip up the form • Simply write VOID through it • Keep the form on file, in your Controlled Substance Book • Try again….

  12. CSOS (Controlled Substance Ordering System) • Allows for secure electronic transmission of Schedule I-V controlled substance orders without the supporting paper Form 222. • Must have DEA number to enroll (Form DEA-251) • Can include C2-C5

  15. Medications have Arrived • Attach invoice to DEA 222 form which was saved • If a medication is on back-order, attach the sheet to the DEA 222 form. Write canceled in the area where with the date and quantity • Log medication into daily RX count books • Place this paper work in a plastic sheet in the binder • Make sure to keep forms in order
