  5. 正常心脏血液动力学 The blood dynamics 右心房 左心房 上下腔静脉 左心室 右心室 肺静脉 主动脉 肺动脉 肺循环 体循环 左心室 右心室

  6. 血液动力学改变 The blood dynamics change 右心房 左心房肥大 分流 肺动脉(扩张) 右心室血量增多 左心室肥大射血量减少 肺循环充血 右心室扩大 体循环(供血不足) 分流

  7. 临床表现 Clinic performance 1 症状Symptom ①生长发育缓慢、乏力、 多汗、 运动耐受 性差、 喂养困难。 ②肺部感染增多、易合并心力衰竭。 ③活动能力下降、 青紫、杵状指(Clubbing).

  8. 2体征 Sign ① 望诊：心前区隆起 、心尖搏动弥散。 ② 触诊：在胸骨左缘第3—4肋间可触及震颤。 ③ 叩诊：心界向左下扩大,晚期向双侧扩大. ④ 听诊：在胸骨左缘第3—4肋间可闻及响亮、 粗糙的Ⅱ—Ⅴ/Ⅵ级收缩期杂音，向四周传导，并有肺动脉第二心音亢进。

  9. 实验室检查 The laboratory check X 线 检 查 心 电 图 超声心动图 心 导 管

  10. 心 导 管 ☆血氧资料:平均血氧饱和右心室上、中部血氧含量明显升高. ☆ 压力资料:肺毛细血管动脉压力升高,右心室压力增高 .

  11. 治 疗 Treatment 1 小型室间隔缺损，不限制体力活动 ， 可以不进行外科手术。 2 缺损较大的，有 临 床症状的室间隔缺损，可以手术治疗或介入治疗。
