  2. Introduction 代謝症候群(Syndrome X, metabolic syndrome, insulin resistance syndrome) • 它是描述聚集在一些病人身上的多重風險因子,每一個因子單獨存在時都會增加罹患心血管疾病(cardiac vascular disease)的風險 • 分為冠狀動脈疾病、腦血管疾病、末梢血管疾病

  3. 第二型糖尿病(Type 2 diabetes) • 又稱非胰島素依賴型糖尿病,胰島功能不足導致胰島素相對性缺乏和對週邊組織產生insulin resistance • insulin resistance是Type 2 diabetes主要發生的原因 • 降低insulin敏感度、增加肝醣釋出,同時產生glucose tolerance異常及hyperinsulinemia,使血糖濃度過高,形成葡萄糖毒性作用,而hyperinsulinemia會增加游離脂肪酸濃度,形成脂質毒性作用

  5. Glibenclamide • 臨床上所使用胰島素分泌劑的一種 • 與胰臟β-細胞受體具有高度的親合力,可提高β-細胞對葡萄糖的敏感性進而促進胰島素的分泌,但並不會刺激胰島素的合成

  7. Materials and methods Patient • 使用24名 Type 2糖尿病患者 ,分為2組,每組各12人 • 年齡限制:40~60歲 • 體重限制:50~70公斤 • fasting blood glucose > 150mg/dl • Glycosylated hemoglobin concentration > 12%

  8. Preparation of the sample and dosage • 將第四、第五片的桑椹葉用水洗淨,並去除不相關的雜質 • 把葉子烘乾至粉末狀,裝在500mg/capsule 的膠囊裡 • Mulberry therapy:一天服用3次,每次2顆500mg/capsule的膠囊,總劑量3 g/day • Glibenclamide:每天服用一片藥片,總劑量5 g/day

  9. Study protocal • 停止一切有關糖尿病的藥物治療,將可能變化的結果降到最低 • 規定要食用同樣的食物 • 食物比例:protein, fat, carbohydrate(20:20:60) • 在四星期後,收集所有病患的lipid資料

  10. Assay methods • 使用autoanalyzer的glucose oxidase and peroxidase method去測plassma glucose • 使用Buege and aust方法去測lipid peroxides and urine

  11. Statistical analysis • 將得到的數據利用t-test去分析所有sample的差異

  15. The results clearly indicate that mulberry therapy is highly effective against lipid peroxidation when compared with glibenclamide treatment. • Whereas glibenclamide treatment showed significant effect on plasma and urinary peroxides, membrane peroxides remain unaffected.

  17. Mulberry therapy significantly decreased membrane cholesterol and phospholipids of type 2 diabetic patients. • However, no significant effect was seen on the membrane cholesterol and phospholipids with the glibenclamide treatment.

  18. Discussion • Mulberry therapy比glibenclamide 更可以影響Type 2 糖尿病患者的glycemic control • Mulberry therapy可以增加胰島素的敏感度 • 可以降低LDL-cholesterol • 增加HDL-cholesterol • Mulberry萃取物可以有效抑制人體的oxidative modification • 使用Mulberry therapy會同時減少紅血球和血液中的cholesterol and lipid peroxides

  19. Conclusion • Mulberry therapy 是有能力去提高glycemic control • Mulberry therapy 可以影響Type 2糖尿病患者的serum and erythrocyte membrane lipids • 臨床實驗必須透過長時間的觀察和討論
