The Qwest Vision • Qwest is a premier provider of full-service communications for people at work, at home or on the move. Qwest's competency reaches across America, drawing on the Spirit of ServiceTM at the heart of our heritage as we effectively steward our shareowners' investments and provide outstanding service to the customers we serve.  As our dedicated professionals move and manage information, we do so with integrity, superior value and advanced products and services, assuming only those tasks we can do exceedingly well.   • Qwest Values • The following values guide our decisions, both corporate and individual, as we transform Qwest into a premier telecommunications provider that delivers exceptional service to customers, creates pride among our employees, delivers value to our shareowners and earns high regard from our many constituents: • Customers come first • Our team is united • We will grow profitably

  3. Release 9.1 – QORA ASR Gateway • Qwest will implement a new ASR Status Tool which will be integrated within the existing QORA GUI application • The ASR Status Tool will display the current status of the ASR, not a historic view • ASR Status is available for ASRs submitted via NDM, UOM, and the QORA GUI • Un-submitted ASRs are not included • Manually submitted ASRs (email, fax) are not included

  4. Customer Impacts/Value • How does implementation of Qwest ASR Status Tool add value for you, our customer? • You can receive current status on ASR’s submitted to Qwest on-line without making a phone call. • Depending on the criteria used for the search, a single ASR may be retrieved or a list of ASRs within a determined time frame. • Links are available on the status summary page to the current Qwest issued industry forms, e.g., Clarification/Notification Request (C/NR), Confirmation Notice (CN), and Design Layout Report (DLR). • For ASRs submitted via the QORA GUI, a link to the most current version of the ASR is also available.

  5. General Changes • QORA ASR Status Tool Access • For current users that only use the QORA GUI for pre-order, a new tab will be available called “GUI Status Tool”. • Users of the QORA GUI for order submission access the ASR Status page under the “Reports” tab. • Customers that do not currently have access to the QORA GUI must contact their Service Manager to request access

  6. General Changes-Cont’d • ASR Status Retrieval Options (Search Criteria): • Customer may input the CCNA and a specific PON to retrieve status on a specific request • Customer may input CCNA and date range to retrieve a list of orders within a particular time frame • ASR Status options are selected via a dropdown list which includes Submitted, In Process, Sup Required, Confirmed, Canceled, Completed, and Rejected for initial submission only. • Ascending or Descending order is selected from a dropdown list • Default date range is 90 days in the past but may be overtyped • A drop down is available with PON “starts with” or “contains” which may be used to narrow the search and must be used with a date range and CCNA • Initiator may be populated to narrow the search when used with a date range and CCNA • When using the date range, customer may select either Date Submitted to Qwest or Date Last Updated

  7. General Changes-Cont’d • ASR Status Retrieval Results: • Only the most current version of an ASR Status is returned. If searching using a date range and the most current version is not found within the date range selected, a message of “Not Found” will be displayed • Rejected versions are not displayed unless the most current version is the initial submission of the order and it was rejected • After populating the search criteria, a GUI Status List is displayed • The PON field in the Status List is an active link to a summary view of the ASR
