CHIP Registration Process • You decide to take the program (or not) • Complete the registration forms • Pay registration fees (prepay or deposit) • If financial assistance is needed, complete a scholarship application form • Select heart screen #1 time slot • Receive forms to be completed at home before heart screen #1 Lifestyle Health Education, Inc.

CHIP Heart Screen Process • Come on time for heart screen appt in fasting state (no breakfast! we feed you) • Bring completed forms with you • Arrange any remaining amount due • Complete each heart screen station • Eat a heart-healthy breakfast • Receive tote bag with CHIP materials • Attend first CHIP session on Monday Lifestyle Health Education, Inc.
