  3. Workshop Structure Who can attend? • Adults with chronic conditions or chronic pain • Adults with prediabetes • Adults with diabetes • Adults caring for someone with the conditions above • Once a week for 6 weeks • 2.5 hours sessions • FREE • Facilitated by 2 trained leaders • Evidence-based

  4. Workshop Objectives & Topics • Action Planning, Decision-Making, & Problem Solving • Communication • Healthy Eating & Physical Activity • Working with Health Professionals • Breathing Techniques • Using Your Mind • Sleep, Medications, and Weight Management • Understanding Emotions

  5. Workshop Information • Located in community and clinical settings • Local program coordination • Referral from hospitals, clinics, and community-based organizations • Registration online or by phone/email
