  4. SIDE 1 – STUDENT & EXAM DETAILS

  5. Write your Family Name first and then initials of your other names like this. Your Family Name is the name that appears first on your ID card and Attendee Card. C I T I Z E N J A

  6. Use your 2B pencil to colour the circles below your name. The letter in each box must match each letter below your name.

  7. Letters range from A to Z

  8. Use your 2B pencil to colour the circles below your Student Number. The letter in each box must match each letter below your Student Number.

  9. For Student Numbers with only 7 numbers, leave the last space and circles blank.

  10. Use your 2B pencil to colour the circles below the letters. The letter in each box must match each letter below the Subject Area.

  11. Use your 2B pencil to colour the circles below the numbers. The numbers in each box must match each number below the Catalogue Number.

  12. The complete Course Code. All boxes and matching circles must be filled in.

  13. SIDE 2 – MULTIPLE CHOICE ANSWERS

  14. SIDE 2 – MULTIPLE CHOICE ANSWERS For Multiple Choice questions with options ranging from A to E. Read the question on the exam paper. Then using your 2B pencil colour the circle matching the multiple choice option you think is most correct. Ensure the question number matches the answer number on this card.

  15. SIDE 2 – TRUE OR FALSE ANSWERS For questions with the option of a True or False answer. Read the question on the exam paper. Then using your 2B pencil colour the circle matching the true or false option you believe is correct. Option A = T = TRUE Option B = F = FALSE Ensure the question number matches the answer number on this card.

  16. OMR Card Instructions • SUMMARY • To ensure there are no problems with marking your OMR card answers, you must: • Use a pencil or black ink pen (2B pencil preferred); • Write your name, student number and course code in the correct boxes; • Colour the correct circles below the boxes and colour inside the lines; • Mark your answers against the correct exam question; • Colour the circles completely and colour within the lines; • Erase mistakes fully; and • Erase any other pencil marks on the sheet.
