  1. M-73 Mobile Hoisting Equipment and Rigging Standard Changes 2013

Justification • The M-73 Mobile Hoisting Equipment and Rigging Standard has been modified to enhance crane matting requirements, crane inspection checklists and to clarify several requirements to address suggestions/feedback from past issues and incidents. Background: • The information provided in this document is a result of cumulative efforts between Dow EH&S, Crane Subject Matter Experts, Crane Contractor Companies and Civil Engineering across North America. Crane providers are in agreement with the changes and are prepared for implementation.

Team Activities • Cross examination of proposed M-73 revisions with new Dow U.S. Crane Safety Standard • Reevaluation of Crane Matting as a result of cumulative efforts between Dow EH&S, Crane Subject Matter Experts, Crane Contractor Companies and Civil Engineering across Dow Texas Operations. • Review of past suggestions/feedback to TXO Site Mobile Hoisting Steering Team

Summary of Significant Changes • Crane Mat requirements and Ground Condition definitions have been completely reevaluated and have the following significant changes: • Mat Matrix changes • New crane capacity ranges • Minimum square footage requirement for capacity ranges • Options for matting clarified for each crane capacity range • Clarified specifications for Timber mats, Steel mats and laminated plywood. • Definitions for Improved Surface, Stable/Improved Soil and Unimproved Surfaces.

  5. Clarifications/Changes • Site Critical Lift Approvers must be designated by Site Rigging Dept. Leader. • Previous Training/Certification Requirements Matrix was removed and rewritten in a more understandable format. No change to requirements for Crane Operator, Rigger, and Signal Person roles. • Dedicated Spotter(near power lines) training requirements was added to reflect OSHA 1926.1408 and TXO Freeport Electrical Work Practice No. 13. • Clarification on requirements for anti two-block exemptions. • Added requirements for Loading Crane Accessories on Trailer to allow workers access only when loads are less than 1ft above landing surface. • Improved Daily/Monthly Inspection for Conventional/Track Crane and Hydraulic Rough Terrain Cranes. • Improved Personnel Hoisting Checklist.

  6. Old Mat Matrix • Old mat matrix Less than adequate guidelines for various types of crane capacities to meet ground bearing pressure requirements.

  8. New Mat Matrix • Clarified specifications for Timber mats, Steel mats and laminated plywood. • Crane outrigger mat minimum square footage is restricted to the following matrix. If available mat is not on the mat matrix contact Crane SME or Rigging Department Scheduler at 979-238-2581 • Timber constructed mats (8" thick) require 1" steel dowel rods with a minimum spacing of 2 feet apart. • All Square and Ø Round mats 4’x 4’ and smaller must be constructed of a minimum of three - 3/4“ ply laminated treated plywood , with some means of designed • Cranes used for hoisting personnel must have a minimum of 4' x 8' x 8“ or 6' x 6‘ steel plate or better • If matting size used is greater than 4' x 8' x 8’‘then a steel transition plate is placed under outrigger pad. • Minimum steel plate thickness = 1 inch • Outrigger pads are required to be in the center of matting, if different configuration required contact authorized by Crane SME or Rigging Department Scheduler at 979-238-2581. • Crawler cranes while under hoisting conditions shall use steel and/or timber mats arranged perpendicular to tracks unless authorized by Crane SME or Rigging Department Scheduler at 979-238-2581. • When building a pad, a minimum of compacted crushed limestone rock must be used to level and stabilize the supporting sub-surface.

New Mat Matrix • Ground Condition Definitions: • Improved Surface –concrete area (6” thick or greater) or plant main roadway. • Stable/Improved Soil – Any area that has high vehicular or equipment traffic such as gravel roads, established asphalt roadways, and compacted lay down areas. • All other surface not included in Improve or Stable/Improved Soil are considered Unimproved Surfaces

Summary of Changes Scope & Applicability • The addition of the equipment below are excluded from M 73 standard • Bridge Cranes, Gantry Cranes, and Semi-Gantry Cranes • Tow Motors with hoisting attachments Roles and Responsibilities • Critical Lift Approvers • Must be designated and approved as Critical Lift Approver by the Facility Leader or by Site Rigging Dept. Leader. This approval shall be documented on the facilities delegation of authority document

  11. Summary of Changes Training/Certification • Previous Training/Certification Requirements Matrix was removed and rewritten in a more understandable format. No change to requirements for Crane Operator, Rigger, and Signal Person roles. • Critical Lift Approver • Facility Critical Lift Approver shall complete Texas Operations M-73 Mobile Hoisting Standard (GHRIS 281530_1095) • Site Critical Lift Approver will be designated by the Dow rigging Department Leader. • Dedicated Spotter (near power lines) • To be considered a dedicated spotter, the requirements of § 1926.1408 (Power line Safety (up to 350kv) equipment operations) and Electrical Work Practice No. 13 must be met. His/her sole responsibility is to watch the separation between the prohibited zone and the equipment as well as ensure, through communication with the operator, that the applicable minimum approach distance is not breached.

Summary of Changes Inspection Requirements Inspection of Anti Two Block • Cranes used in limited head room situations where the anti two-block device may be removed must have an approval granted by the EHS Delivery Leader of that department prior to removing or bypassing the device. Upon completion of the task, the anti two-block device must be immediately re-installed and checked for proper operation. • Anti two-block devices shall be operable on all cranes.

Summary of Changes Anti Two-Block Exemptions Circumstances where the anti two-block device may be removed must have the following: • Approval granted by the EHS Delivery Leader of that business prior to removing or bypassing the device. • Clearly mark the cable (so that it can easily be seen by the operator) at a point that will give the operator sufficient time to stop the hoist to prevent two-blocking (minimum of 10ft above the becket), or use a spotter. • Upon completion of the task, the anti two-block device must be immediately re-installed and checked for proper operation.   14. Summary of Changes • Loading Crane Accessories on Trailer (Ex. Counterweights, Block, Ball, etc.) • If the truck to be loaded does not come equipped with ladders permanently attached to the side at the front and rear of the truck, then stepladders will have to be obtained and tied off to the sides of the trailer of flatbed. The stepladders will be opened fully and set for easy access to the truck • The worker can access the deck to position the crane accessories when the load is < 1ft above the trailer/flatbed./landing surface.

  15. Other Changes • Improved Daily/Monthly Inspection for Conventional/Track Crane and Hydraulic Rough Terrain Cranes. • Improved Personnel Hoisting Checklist.

  16. Q&A • Q1. Is this change in mat sizes going to cost Dow Riging Dept. and Contractor Companies a signifigant amount of money? • A1. The changes have been reviewed with Dow Rigging Dept. and Crane Companies and they are comfortable with the cost impact. • Q2. Why did we change the mat sizes, they’ve been the same for 10+ years? • A2. The Dow Global Civil Dept. released new ground bearing pressures for sites Globally. Though we have not had incidents with the current mat sizes, it was decided based upon the new data and evaluation by SMEs that some of the previous mat sizes needed to be updated to ensure appropriate safety factors.
