  4. Part A Online Form Enter your name EXACTLY as it appears on your Government Issued ID. You will receive your USMLE ID and password via email in one business day.

  5. Completing the USMLE Step 1 Online Application Your Name! If you have middle name on your government issued ID, use it on your USMLE Application. Good to go! • Tommy T.   6. Testing Window & Scheduling Permit Pick a three month testing window: • May June July • June July August • A one time extension is possible for a fee • Receive scheduling permit by email. You will need the information on your permit to schedule the exam.

  7. Testing Accommodations • Indicate that you are applying for accommodations in your application. • APPLY EARLY! • Read the Request for Test Accommodations for Examinees with Disabilities information, which can be downloaded from the USMLE website.

  8. Part B Verification FORM Bring this form with a picture to the Office of Student Affairs, KAM 100 D. You will mail this form to the NBME with your $$$.
