Accident investigation process prior to the implementation of the Safety Directive • What organisations were involved? • Infrastructure Maintainer • Railway Undertaking • Railway Inspectorate • Railway Regulator • Railway Accident Investigator • Other Bodies • Police • Judiciary • When was this structure in place? (from date to date) • Was legislation governed this structure? • What was the purpose of the accident investigation?

Accident questionnaire for all organisations in the country going back to 1990 (1) • Name of Investigation Organisations • Date Formed • Was there a legal requirement to investigate certain types of accidents? • Were investigations carried out by an independent body? • Which type of accidents were investigated? • Collisions • Derailments • Collisions between trains and road vehicles • Train Fires • Accidents involving dangerous goods • Accidents involving significant damage • Illegal or unauthorised acts • Falls from moving trains • Are there other type of accidents / incidents not covered above that were investigated?

(body publishing them, website) • If so, by whom? • What is the oldest investigation report that you know of? (year, if known) • Where are the investigation reports kept?

Accidents Identified for Bulgaria • 18/08/92, Kazichene station • A freight train travelling at 100 km/h went through a stop signal and was hit by a passenger express train travelling from Burgas to Sofia • 8 fatalities, 57 serious injuries • Railway Undertaking BDZ, Infrastructure Maintainer NRIC • 01/10/1992, Harmanli • An international passenger train travelling from Istanbul to Munich collided head-on with a Liubimez-Biser freight train • 3 staff, 2 passenger fatalities • 1 serious injury • Railway Undertaking BDZ, Infrastructure Maintainer NRIC

  12. Availability of Accident Statistics and other Information • What Bulgarian railway accident statistics have been published? • How have these statistics been collated? • Is there a central record of safety-related incidents (e.g. the UK SMIS system)?

  13. Feasibility of Extending the Database back in time • What formal accident reports have been published? • Are they readily available in electronic / paper form? • What language are they available in? • Are there other sources / reports / websites / papers available to support the project?
