  3. Program Goals • Reduce maternal and neonatal complications by early assessment, prevention, and early detection of risk factors/conditions • Reduce admissions or length of stay in neonatal intensive care unit • Reduce the incidence of pre-term deliveries and low-birth weight infants

  4. Participant Identification • Claims data • Self-referral • Provider referral • Referral from other Wellmark health and wellness programs • Referral from IBBP Customer Service agents

  5. Preconception Education • One educational postcard • Mailed annually to women ages 18-40 • Emphasizes importance of healthy lifestyle • Identifies assistance and resources available through Pregnancy Care program

  6. Risk Level Identification • Initial assessment identifies lifestyle or pregnancy related risk factors • Participants segmented into three risk levels • Low-risk • At-risk • High-risk

  7. Outreach for Low-Risk Participants • Initial telephonic assessment • Inbound calls 24/7 up to six weeks postpartum • Mailed educational material • (welcome letter and workbook) • Monthly mailings customized by • gestational age of baby • (English and Spanish)

  8. Outreach for At-Risk Participants • All levels of outreach included with low-risk • AND • Lifestyle behavioral change support based on smoking, stress management, depression, and healthy eating goals • Assessments for risk level via phone • Postpartum call to assess outcomes and postnatal depression screening

  9. Outreach for High-Risk Participants • All levels of outreach included with low-risk and at-risk • PLUS • Primary OB nurse model with proactive nursing contact monthly or more frequently based on risk and compliance with treatment • Collaboration on treatment plan with health care professional

  10. Participation Gift • $100 gift card • Use anywhere • Awarded after initial telephonic assessment
