The standardized Formulary request form will streamline the revision process and improve data accuracy for an improved user experience Background The MSPV-NG Formulary Management Team (FMT) receives email requests daily to make updates to Formulary data. It is important that changes are made accurately to reflect existing contract data so users are receiving their medical/surgical requirements on time and for the best value. Problem The FMT currently receives requests to update the MSPV-NG Formulary in variety formats. Lack of standardization does not always provide clear instructions or all of the information needed to maintain accurate data. Solution The FMT has created a standardized form that will ensure the requestor provides all information and context needed to make the changes that are intended. It will also make the revisions process more efficient by reducing the necessity for the FMT to reach back out to the requestor with clarifying questions.

  4. Find the most updated Formulary request form at the MSPV-NG website https://www.va.gov/opal/sac/mspvNG.asp

  5. Scroll down to the “Change Requests” section and double click on the “Formulary Item Request Form” to open a blank form Click to open email to submit a request for additions to the Formulary Click to open the most recent Formulary listing Click to open email to submit a correction to the Formulary Click to open a blank Formulary request form

  6. Determine which request type is appropriate by using the guidelines outlined in tab 1 • 1. How to Request Revisions to the MSPV-NG Catalogue • Use the "Revisions" tab for requests related to currently listed Catalogue items, including: • -Correcting outdated or incorrect product data • -Identifying a replacement to a current or removed item from the MSPV-NG Catalogue • -Adding item(s) in the same product suite as current Catalogue items– • Items will be considered for addition if they are: • A variation of a Catalogue item is missing (e.g. size, color) • A part or accessory needed for a Catalogue item is missing • -Adding item(s) that are updated or improved versions of current Catalogue items (e.g. due to technology advancements) • 2. How to Request Additions to the MSPV-NG Catalogue • The Healthcare Commodities Program Office (HCPO) will periodically review requests for additions to the MSPV-NG Catalogue. • Use the "New Item Request" tab solely for new items. See above for all other requests

  9. Select tab 2 to fill out a revision request

  10. Fill in the requestor information Requestor type options are Vendor, VA facility or SAC Facilities will fill in their VISN and facility number Vendors will fill in their name and socioeconomic category

  11. Add POC information and rationale for request Explain exactly what change is requested by listing each column that should be updated and to which prime vendor(s) the change applies Add name and email address of requestor/individual who can answer any questions about the request Request type options are revision or removal

  12. Enter item information currently on the Formulary Ensure any field marked (Required) in red is filled out Include the information that is on the most recent Formulary publication. This will allow the FMT to accurately identify the item that needs a revision or to be removed.

  13. Under the “New Product Information” section, add data ONLY to columns which need to be updated • This section will be blank for removal requests • Example: Only the price column would be filled out for a price change

  14. Send the completed request form to VHAMSPVCorrection@va.gov by clicking the “Email form” button at the top of the tab Note: revision requests received by the 7th of the month are generally reflected in that month’s publication Click to open an email draft with completed form attached

  16. Select tab 3 to fill out an addition request
