Action Plan #1 NCM Procedure Applied • Flasher Board STF Failures (18% of Fails): The largest NCM contributor • Root Cause(s): • Main Board Clock Driver behavior • Other potential causes under investigation • Corrective Action: • Revised HAL software/Flasher Bd. CPLD firmware • LBNL investigating MB Clock-possible new part. • “low risk” changes for this year • Prognosis for PY4: • Significant reduction in occurrence. • Watch in Mini-Fat.

Action Plan #2 NCM Procedure Applied • Low PMT Gain Failures (14 % of Fails): Second largest NCM contributor • Root Cause(s): • Software Glitch in testing • Same Gains specs applied to both Ice Top & In-Ice PMTs. (10% PMTs right at Gain Margin) • Corrective Action: • Instituted Software changes for FAT3 • For PY4, Incoming Insp screen to sort lower end gains for Ice Top/ higher end gains for in-ice. • Prognosis for PY4: • Elimination of Low PMT Gain failures

Action Plan #3 NCM Procedure Applied • Reboot Failures (14 % of Fails): Third largest NCM contributor • Root Cause(s): • Initial CPLD programming not robust • Other Root Cause(s) under investigation • Corrective Action: • Reprogrammed CPLDs/ Retested DOMs. Works for most. • 3 “4-29 Rev. CPLD” failures in FAT 3/ Re-run in FAT 4. • Further investigation continues. • Prognosis for PY4: • No new reboot failures in PSL FAT 4, DESY FATs 2, 3. Looks good for this year.

Action Plan #4 NCM Procedure Applied • OTHER: 12% of Failures • Fourth largest NCM contributor • 7 = DC Corning Gel • 2 = Incorrect DB Entries • 2 = Monitor DAQ • 2 = REDO DOMCAL • 1 = Many STF Fails • 1 = Chipped Sphere • 1 = Broken Penetrator Connector • 1 = Pressure Leak • Prognosis for PY4: • No more DC Gel • Break out “Other Category”

  8. Going Forward • We appear to have solved the “Big” reasons for Fallout (80%) • However: Increased production • New, Faster Evaluation Methods • Keep Production Going and return vital analytical info to Project • Supplier Communication: Vital • Open up NCM process to ALL!
