  3. Forms Capture Amount of Direct Coast Guard Support - Boat & Air Patrols - Instruct Recruits - Radio Watchstander - Administrative - Vessel Safety Checks - Public Education - Program Visitors

  4. Forms Capture Amount of Indirect Coast Guard Support - Member Training - Public Affairs - Meetings - Recruiting - Academy Intro Mission - Administrative

  5. Forms ……… Not only capture our assistance to provide funding data Provide measurements of Flotillas, Divisions and Districts Justify awards to members

  8. 7028 Change of Member Info You should be submit when anything changes. In section 2 Enter only changed items.

  9. 7029 Member Activity Meetings, Training, Elected or Staff Duties, Prep or Travel for Activities on other forms

  10. 7030 Mission Activity Report This captures Mission Related activities: - Marine/Air Patrols Cox/Pilot submits - Public Education Class Mgr submits - Marine Safety - Marine Envir. Prot. - CG Operational or Admin support - Radio Net - Member Training - Public Affairs

  11. 7038 Vessel Exams • For all exams • Vessel Safety Checks • Facility Inspections • Commercial Fishing • For VSC’s, attach the • 204/7012 forms • You should submit • at least monthly

  12. 7039 Workshop Used to report all National Workshops Instructor and Aids get MT Hour credit Attendees get credit as Trainee’s Must be a qualified Instructor.

  13. 7046 RBS Visits (Marine Dealer Visitation) You can report up to 15 Visits on this form. Should be submitted at least monthly

  14. Form Flow - Forms are completed by the member - Member submits to FSO-IS - FSO-IS checks for completeness and errors, and sends to the SO-IS - SO-IS inputs to AUXDATA - Once a week, AUXDATA is rolled into AuxInfo - Member can check their data using AuxInfo
