    4. Facilities, Materials, Equipment, & Transportation Outdoor play area allowed access to an unsafe/unsupervised area. Unsafe items in open storage Aluminum easel tray with sharp corners at a childs eye level Items stored in unlocked cabinets were unsafe and accessible to children Failure to provide emergency lighting 8 non-compliances were corrected onsite, 3 deficiencies were corrected onsite. Although corrected onsite, they are all noted in the final PRISM reports. 1304.53(a)(7)(9) Deficiency NHA 1304.53(a)(7) ANC Santa Cruz Community Counseling Center Unsafe items include rusty gardening tool, shovels, rakes 1304.53(a)(10)(iii) ANC Southwest Human Development and Deficiency Hawaii County Economic Opportunity Council The ANC was for items including plastic bags, shampoo, long thin paring knives, shaving cream, and other items stored in an open cabinet above a classroom counter. The deficiency was for Clorox, liquid detergent, disinfectant cleanser, Borax and plastic garbage bags stored in unlocked cabinets accessible to children 8 non-compliances were corrected onsite, 3 deficiencies were corrected onsite.
