  4. Success Planning • Respectful supports focus on the physical, emotional and mental well being of the person • What tool in your toolbox can you use for this?

Success Planning Day • Any discussion that focuses on the negative side, how bad or non-compliant the person is will not be an option for today’s discussion • All discussion & sharing will focus on solutions that lead to personal success and improved quality of life • NO quiet as a fish for the person

  7. Identify for a Success Plan • The things that we appreciate & value about this person • The strengths of this person • What this person contributes to other people, including family, friends, co-workers, paid supports

  8. What tool in your toolbox can you use to identify all areas of the person’s life and the positive contributions?

  9. Identify • What are the best memories of this person. • What can we do to help this person to have a “ Good Day”.

  10. This means each person will list how they (personally) use their Ears- Eyes- Hands- Heart- Thoughts- Time –Words • What tool in your toolbox can you use for this?

Identify • Things that are their “Least Favorite” • Things that are their Favorites (People-Places-Things) • Tool Box Do’s & Don’ts by identifying the support profile ( What works and what doesn’t) • Identify and list the commitments & responsibilities that the CB & provider will agree to that will enable the person to have a Good Life.
