RURAL HEALTH PROGRAM FUNDAMENTALS • Comprehend – Engage the community, its leaders and providers to better understand the needs of our Veterans. • Collaborate – Engage those Veterans encountered, their families, and other VA consumers. • Complement - Focus on health issues among Veterans to address commonalities • Partnerships – Build to improve screening, health maintenance and compliance..quality of life.

GOAL #1OUTREACH • Enrollment via engagement of Veterans in their community or surrounding areas. • Registration – assist with the completion of form 1010 for healthcare. • Primary Care and Mental Health service connection. • Provide additional information and assistance on form 21-526 (with the support of VSO offices). • Assist in the navigation of the Department of Veterans Affairs system……or at least refer to the appropriate department. • Reach out to as many Veterans in Rural American as is possible.

GOAL #2EDUCATIONAL ENHANCEMENT • Increase the veteran’s knowledge of, compliance with and awareness of chronic diseases such as Cardiovascular Disease (CVD), Congestive Heart Failure (CHF)and Diabetes Mellitus (DM). • Present and provide education and guidance on healthcare information that is geared towards the 9 healthy choices as defined by VHA National Center for Health and Disease Prevention.

Goal #2EDUCATIONAL ENHANCEMENT (CONT.) 9 HEALTHY LIVING MESSAGES: • Be involved in your health care • Be Tobacco Free • Eat Wisely • Be Physically Active • Strive for a healthy weight • Limit Alcohol consumption • Get recommended screening tests and immunizations • Manage stress • Be safe • Your Choice

GOAL #3COMMUNITY PARTNERSHIPS • Identify and partner with community organizations to create forums in the rural areas for health education classes, programs and outreach. • Identify and partner with community leaders to ensure that Veterans have every opportunity to maximize their health outcomes through VHA, VBA and NCA services.

Outreach Efforts thru Oct 2010 thru July 2011 (Female Only) Total Number Rural Veterans Encountered: 1904 (159) Total # Rural Veterans Engaged: 1243 (158) Number of completed applications received: 633 (39) Number of new enrollees*: 432 (18) Number schedule but not seen yet: 88 (4) Number of newly seen and now vested: 344 (13) Performance (Total Vested/Total Enrolled): 80% *Enrolled AFTER Documented Contact with RH Team
