  1. Ref no: 04062011001 QHSE ALERT On 4th June 2011 at 9:01 am, convoy of 4 vehicles, two explosive and two contracted Commander vehicles left the Explosive storage site (Amarpur). Midway at Varmatila, near Devipur between Shanti Bazaar and Amarpurthe front commander vehicle and the public transport commander vehicle had a head-on collision. Blind curve and downhill road • 5 Tripura State Rifles men (TSR)+driver and 3 civilians were injured. • 7 of the 9 injured, were given first aid at Amarpur and discharged and 2 were transferred to Udaipur hospital via the Asian Oilfield ambulance. • Total 5 TSR+1 driver and 18 civilians+1 driver who ran away just prior to the collision were present in the 2 commander vehicles (3 civilians were on the top of the vehicle – not injured.) • Lessons learnt: • Blow horn and reduce speed at turns. • Expect the unexpected especially at blind turns.
