Integrated Healthcare Holistic diagnoses Fewer patient visits=increased capacity and cost effectiveness Prescription of cost appropriate medication=better medication compliance Comprehensive medication monitoring-medical and behavioral health medication Patient centered treatment-no “shoulds” Universal screening for behavioral health issues Team approach to treatment-joint appointments Immediate “warm referral” to behavioral health services Linkage to resources and external support Outreach and follow up High patient engagement in treatment Measured outcomes

    6. How do we move from Fragmentation to Integration? Incentivize and monetarily support integrated care in community health and mental health settings Clarify the ability of Medicaid providers to bill for mental health services and primary care services provided on the same day. Include primary and behavioral healthcare integration in healthcare reform efforts such as medical homes. Require that persons with SMI be a mandatory target population in chronic care programs; need parallel Medicaid Demo.
