  2. 55 yr old male , from Wrigleyhas DM – 20 yrs , HTN – 1 yrpresented with progressive rise in s.Cr: Dec 05: 112 July 06: 289 Aug 06: 256 Sep 06: 281 Oct 06 : 350 Dec 06 : 473 Jan 07: 543c/o : leg cramps, tiredness , Bp: 150/85Lab : urine: 3+ ptn, 3+ Hb ptn/cr : 290 pANCA +ve C3, C4 , ANA, anti-GBM : -ve Hb 85 , Meds: atacand, atenolol, lipitor, rocaltrol, CaCO3, insulin

  3. IF • IgG-Negative. • IgA-Negative. • IgM- Mild vascular staining. • C3- Moderate vascular staining. • C1q-Negative. • Kappa-Negative. • Lambda-Negative. • Fibrinogen- Moderate staining of necrotizing glomerular lesions, otherwise negative. • Albumin- Mild linear TBM staining, a common finding in diabetes.

  8. DiagnosisRenal Biopsy: • Pauci-immune necrotizing glomerulonephritis with crescent formation • Superimposed on moderate diffuse and nodular diabetic nephropathy. • Possible allergic or infectious interstitial nephritis
