Back to Main Page LTI Notification LTI No 01 2013 8th January Accident Location: Fahud PAC WHAT HAPPENED: The IP was in his PAC room’s bathroom when scalding water poured down onto his neck and back from a ceiling light fitting. Water from a leaking water pipe in the roof space had seeped through the ceiling via the gap for the lights electrics and accumulated in the lights bowl shaped lamp shade. The heat from the light bulb then heated the water to such a temperature that it melted the plastic shade and poured down on to the IP who was standing directly underneath at the time. He was treated locally and then transferred to Ibra hospital for further treatment. • INJURY: • Scalding injury . • 5 Work Days Lost. Hole for the cables in the light fitting giving the water a path to flow through the ceiling Example of a lights lamp shade

Back to Main Page LTI Notification LTI No 02 2013 20th January Accident Location: Saih Nyhaida area WHAT HAPPENED: The IP was placing a shackle on to a casing insert bowl in order to relocate it from the catwalk back to the equipment store.  Whilst he was tighten the shackle over the 1st casing insert bowl, the bowl slid down  and this trapped his right middle finger between tow bowls. The IP was treated locally at the campsite clinic and then referred to Nizwa hospital for further treatment.  A fracture to the tip of his finger was confirmed. • INJURY: • Fracture to the tip of his finger . • 5 Work Days Lost. Back to Main Page LTI Notification LTI No 03 2013 3rd January Accident Location: Rig-52, KCA Deutag, Fahud WHAT HAPPENED: The IP was mixing chemicals in the mixing hopper area and as he was lifting a 25 kg chemical bag, he lost his balance and fell 2 metres from the top of the mixing hopper area to the ground.  He was taken to rig medic and then to Fahud PDO clinic. IP was referred to Nizwa Hospital for X-Ray & further examination showed he had fractured his right forearm. Hopper on rig Distance IP fell • INJURY: • Arm fracture • 16 Work Days Lost.

Back to Main Page Fatal LTI Notification LTI No# 04/06/07/08 2013 10th February Accident Location: Rima Graded road near Wifaq WHAT HAPPENED: Four employees were in the vehicle travelling from Ghanima to Ghubrah village when their vehicle rolled over at a turning nearby the Wifaq well location and resualted; LTI#4 (Fatality) 27 year old QC Civil inspector LTI#6 Ankle fracture LTI#7 Chest pain LTI#8 Neck Wounds Driver and passengers working for Jalmood National a contractor for Petrogas, a PDO ‘small fields’ contractor. Back to Main Page LTI Notification LTI No 05 2013 8th February Accident Location: Rig 36 Lekhwair WHAT HAPPENED: The IP was working on the iron roughneck which had been previously removed from the drill floor as bolts holding the pistons to the roughneck body had sheared. With the use of a webbing strap, the IP was supervising reintroducing the top assembly of the roughneck onto its base assembly and engaged the back pin successfully into its slot.  The IP then attempted to rotate the upper assembly to allow it to drop down onto the two front pins. In doing so the IP placed his fingers in a 4cm gap below the upper assembly pistons and as it successfully dropped on to the pins the gap reduced to less than a 1cm. The very top of his right ring finger was severely crushed. Lifting webbing strap Upper assembly Pinch point which crushed finger between upper and lower assembly Lower assembly • INJURY: • Broken finger. • 12 Work Days Lost.

Back to Main Page LTI Notification LTI No 09 2013 24th February Accident Location: Hoist 35 Shiv-Vani in Lekhwair WHAT HAPPENED: The hoist was in the process of breaking a pipe joint after they had removed the pipe from the utility hole.  The driller and the floor man tried to push the pipe towards the mouse hole but were struggling to push it across.  The IP who had just come onto the rig floor went over to help them.  While walking over to the rotary table his left foot went into the uncovered utility hole causing him to fall over on the rig floor.  He fell on his left elbow which took the full weight of his body. He was taken to the PDO Clinic in Lekhwair and then referred to Ibri Hospital where X rays identified a hairline fracture to his left upper arm. RTA • INJURY: • Upper Arm Injury “Shoulder”. • 30 Work Days Lost.

Back to Main Page LTI Notification LTI No 10 2013 1st March Accident Location: FHD-523 steam well, Fahud WHAT HAPPENED: The Mason was aligning the foundation slipper (size: 1.7 x 1.5 x 1.2 meter) when it hit him on his left foot. • INJURY: • Multiple fractures on left foot. • 14 Work Days Lost.

  9. Back to Main Page LTI Notification LTI No 11 2013 19th March Accident Location: Rig 49_ Dalma in Lekhwair. WHAT HAPPENED: The IP was attempting to lower the mast of a portable tower flood light from its upright position down to its horizontal resting position. The mast stopped lowering at approx. 6 – 8 inches above the mast guide and the wire rope slackened.  The IP caught hold of the wire rope to place it back on the pulley when the flood light mast suddenly dropped down further tightening the wire rope and causing his hands to become trapped between the pulley and the wire rope. IP was immediately taken to the clinic for first aid treatment for a cut and loss of nail to his left index finger. He was later referred by the Lekhwair clinic to Ibri hospital where he was diagnosed with a fractured left index finger. IP trapped fingers when wire rope he was putting back on the pulley by hand unexpectantly tightened causing crush point between wire and pulley • RESULTS: • Fractured left index finger. • 14 Work Days Lost. Wire rope slackened when lighting column stopped lowering

  10. Back to Main Page LTI Notification LTI No 12 2013 20th April Accident Location: Bahja, Sadad 082- well head, Haimo Technologies . WHAT HAPPENED: IP was walking from the mobile well test unit to the crew pick-up carrying two glass sample bottles full of crude oil. As he approached the flow line he turned round (from where he was going) to ask a colleague a question but continued to walk backwards. In doing so he tripped backwards over the flow line. As he fell, he kept hold of the glass bottles to prevent them smashing and in doing so he was unable to protect himself from his fall, leading to him landing heavily on his elbow. The IP was moved to Haima clinic and later transferred to Muscat for further treatment after being diagnosed with a fractured arm. Direction he faces 3. IP lands heavily on his arm, still holding the sample bottles in hand. Direction of travel 1. IP turns to talk to his colleague, while walking, with 2 filled sample bottles in hand. 2. IP trips on flowline losing his balance, and falls over. • INJURY: • Fractured elbow • 6 weeks off work.

Back to Main Page LTI Notification LTI No 13 2013 19th April Accident Location:Lekhwair,IPC Coiled tube unit (CTU21) WHAT HAPPENED: The IP was disconnecting the lubricator from the BOP but he did not notice that the lubricator was still under tension. After manually unscrewing the lubricator, he signaled the operator to lift the lubricator using the hydraulic track stack. The IP had placed his hand between the BOP and the lubricator and as the lubricator was lifted the tension caused it to unexpectedly move sideways causing a crush injury to the IP’s right middle finger The IP was immediately taken to the Lekhwair PAC clinic and then referred onward to Ibri hospital where he was treated for a compound fracture of his right middle finger. Slightly tilted X-mas and BOP Assembly  • INJURY: • Fractured right middle finger • 4 weeks off work. Keep your hands off the pinch points to protect from crush injuries.

  12. Back to Main Page LTI Notification LTI No 14 2013 20th April Accident Location:Nimr, karim West-98 _FBU-6 unit _ Weatherford. WHAT HAPPENED: Whilst the IP was working on the floor of the unit he felt dizzy and fell to the ground from the rig floor through the access stairs, a height of approximately 1 metre. The IP was immediately taken to Nimr Tawoos Clinic, and referred to Haima hospital for X-ray then to Nizwa Hospital where he was diagnosed with a fractured left leg. • RESULTS: • Fractured left leg • 4 weeks off work.

Back to Main Page LTI Notification LTI No 15 2013 20th April Accident Location: Nimr pumping station NRPS , ATE WHAT HAPPENED: The IP wanted to align the level of a steel supporting frame made for the beam bottom which was 2.55 metres above the ground. To do this he stepped on an unsecured wooden plank in place to provide support for the beam but in doing so both the plank and supporting frame slipped causing the IP to fall to the ground. He landed on both legs but the impact resulted in trauma to the spine and was unable to move. His co workers and site foreman transferred him to the ATE clinic where the nurse examined him but could find no visible injuries found. He was further sent to the Haima Government hospital by Tawoos ambulance for further diagnosis where they identified a wedge compression fracture of his vertebra (Spinal )  • INJURY: • Fracture of vertebra (Spinal ) •  3 weeks off work.

  14. Back to Main Page LTI Notification LTI No 16 2013 25th April Accident Location: Harweel,Galfar a subcontractor of Petrofac. WHAT HAPPENED: The IP was re-stacking a number of steel brackets that had been returned from site and stored in a unsafe manner.  While moving one of the brackets another bracket fell down from the stack and trapped the IP's little finger between the brackets. This resulted in a fracture to the IP's right hand little finger. • RESULTS: • Fractured finger • 4 weeks off work.

Back to Main Page LTI Notification 30th May LTI No 17 2013 Accident Location: Burhaan near Qarn Alam, KCA Deutag WHAT HAPPENED: Whilst running in drill pipe and after successfully making a connection the IP went to secure the tongs. In doing this he incorrectly held the tong by its jaw rather than by its handle. Due to the weight of the other jaw it swung and closed by itself and trapped the IPs right ring-finger in between both jaws causing him pain. There was no visible cut or bruise to his finger but the IP was taken to see the Rig Medic who decided to transfer the IP to Nizwa Hospital as a precaution.  At the hospital he was X rayed and diagnosed as having suffered a hairline fracture to his finger.  He was then transferred to Khoula Hospital for specialist treatment. • Injury : • Fractured finger • 2 weeks off work

  16. Back to Main Page LTI Notification LTI No 18 2013 13th May Accident Location: Saih Rawl ,STST, the north EMC contractor for PDO. WHAT HAPPENED :  The IP was helping to align the holes in the excavator arm and excavator bucket to enable the connecting pin to be inserted. Whilst attempting this alignment his thumb on his right hand was crushed between the excavator arm and the bracket on the bucket, resulting in injury.  The IP was first taken to the STST clinic and was diagnosed with a fractured thumb. He was immediately transferred to Adam hospital by the STST doctor and then later transferred again to Nizwa hospital for further specialist treatment. • INJURY : • Fractured finger • 2 weeks off work.

  17. Back to Main Page LTI Notification LTI No 19 2013 25th May Accident Location: Nimr,MB Petroleum Services unit Hoist 22 . WHAT HAPPENED : Whilst the IP was coming out from the work shop he slipped and fell down. He injured his right leg between the knee and the ankle. The IP was immediately taken to Sahara PAC clinic in Nimr, and referred to Salalah hospital where he was diagnosed with a fractured right leg. Step it is believed the IP fell from • Injury : • Fractured leg • 6 weeks off work.

  18. Back to Main Page LTI Notification LTI No 20 2013 31th May Accident Location: Nimr Rig-47,Dalma Energy. WHAT HAPPENED : After making connection, 4” Drill pipe picked up from V-Door to the rig floor prior to lay down into the mouse hole for next connection. In that process a manila rope was used to receive and guide the pipe from the V-Door to the rig floor then the pipe was guided by hand into the mouse hole. While lowering the pipe landed on IP foot, which was close to the mouse hole and resulted in injury on his right foot little toe. The IP was initially seen by the Rig Medic then taken to Nimr PDO clinic. He was then transferred to Sultan Qaboos Hospital / Salalah where he was diagnosed as having a.   • Injury : • Fractured toe • 4 weeks off work.

Back to Main Page LTI Notification LTI No 21 2013 8 th June Accident Location: PDO Fahud main office, Saif Sultan Trading a sub-sub contractor to Khimjis who are themselves a sub contractor to WIPRO. WHAT HAPPENED : The IP was one of a team of carpenters who were attempting to open a wooden pallet box which had been used to transport reinforced glass panels.  As they were prising away the wooden slats at the front of the pallet box it tipped forward and fell on to them injuring two of the carpenters, one seriously. Both IPs were taken to the Fahud clinic where one IP was diagnosed as having fractured his right leg. He was transferred to Ibri Hospital for X-ray and specialist treatment. His colleague was diagnosed with minor bruising and treated as a first aid case within the clinic. • Injury : • Fractured leg • 6 weeks off work.

  20. Back to Main Page LTI Notification LTI No 22 2013 16 th June Accident Location: Bahja,Weatherford Drilling. WHAT HAPPENED : This is to notify staff of LTI#22 which occurred on the 16th of June 2013 at approximately 21:00 hrs involving a 23 year old floorman (IP) working for Weatherford Drilling in Bahja on Rig-04. During the installation of a safety clamp on an 8 ¼” drill collar, a floorman was hammering the safety clamp in place to get the correct alignment.  The IP was helping him by holding the safety clamp but was doing so incorrectly with his left hand on the handle and the right hand on the die holder.  The floorman failed to notice that the IP was gripping the holder and the hammer struck the IP’s finger. The IP was seen by the Rig Medic then taken to PDO Clinic in Bahja. He was later referred to Haima Hospital where he was diagnosed with a fractured finger. • Injury : • Fractured finger • 4 weeks off work.

  21. Back to Main Page LTI Notification LTI No 23 2013 16 th June Accident Location: Al Turki, Ras Al Hamra. WHAT HAPPENED : This is to notify staff of LTI#23 which occurred on the 16th of June 2013 at 11:30 hrs involving a 33 year old helper (IP) employed in a three man surveyor team by Al Turki on the Ras Al Hamra.  After helping the surveyor who had been surveying from up a Jebal, the IP whilst returning to the welfare area for lunch was climbing 1.2m down a set of unsecured ladders into a cutting when the ladder slipped and he lost his balance.  As he fell his leg became trapped between two rungs of the ladder causing the bone in his lower left leg to break. He was rescued by co-workers who were working nearby and a first aider provided first aid before the IP was transferred to the Ras Al Hamra project site clinic.  He was subsequently moved to the Atlas hospital in Al Ghubra where X-rays confirmed the break to his leg and he was operated on to secure the bone back in place. • Injury : • Fractured leg • 4 weeks off work.

Back to Main Page LTI Notification LTI No 24 2013 4th July Accident Location: Yibal, Galfar WHAT HAPPENED : This is to notify staff of LTI#24 which occurred on the 4th of July 2013 at approximately 12:00 hrs, involving a 26 year old Bangladeshi carpenter (IP) working for Al Romal a sub contractor to Galfar operating from the Yibal ODC industrial yard The IP was cutting plywood sheets using a wood cutting machine which had an unguarded rotating cutting blade.  Whilst feeding the plywood close to the blade using his unprotected bare hands, the thumb of his left hand contacted the rotating blade causing a deep cut. He was immediately taken to the Yibal PDO clinic then transferred to Ibri hospital where the tip of his left thumb had to be amputated.  He was further referred to Khoula hospital, Muscat for the further treatment by a plastic surgeon. • Injury : • Amputated thumb • 2 weeks off work. Back to Main Page LTI Notification LTI No 25 2013 4th July Accident Location:Lekhwair,working for Al Moton transport, a sub contractor to Abraj WHAT HAPPENED : This is to notify staff of LTI#25 which occurred on the 4th of July 2013 at approximately 08:00 hrs, involving a 35 year old Indian Plumber (IP) working for Al Moton transport, a sub contractor to Abraj working in Lekhwair. A team from Al Moton had been sent to the new yard to conduct maintenance work in preparation for Abraj to move in.   During the maintenance, the IP used a ladder to access the roof of a portacabin situated within the workshop.  He had however placed the ladder upside down such that the rubber feet were at the top and so the base of the ladder had no grip.   On reaching half way up the ladder, the base of the ladder slipped and the IP fell with it. His leg became trapped between the rungs as he hit the ground. His colleagues working nearby came to his aid and transferred him to Lekhwair clinic for initial medical treatment. The IP was later transferred to Ibri hospital where X-rays confirmed that he had broken his shin bone above the ankle on his left leg. • Injury : • Fractured ankle • 6 weeks off work.

  24. Back to Main Page LTI Notification LTI No 26 2013 10th July Accident Location:PDO Harweel. WHAT HAPPENED : This is to notify staff of LTI#26 which occurred on the 10th of July 2013 at approximately 1000 hrs, involving a 27 year old trainee Mechanical technician (IP) working for PDO Harweel. After finishing his duty at Harweel Main Production Station the IP was driven back to the workshop along with the senior technician in a 3 ton canter. While getting out of the cab the IP lost balance, twisted his right ankle & fell down. The IP was immediately taken to PDO Clinic at Harweel and then sent to SQH-Salalah where the X-ray revealed a fracture to his little toe. • Injury : • Fractured ankle • 11 weeks off work.

Back to Main Page LTI Notification LTI No 27 2013 26th July Accident Location: Saih Rawl Rig-45 Abraj Energy Services WHAT HAPPENED : This is to notify staff of LTI#27 which occurred on the 26th of July 2013 at 03:00hrs involving a 31 year Floor Man (IP) working for Abraj Energy Services in Saih Rawl Rig-45. The IP was opening the safety clamp using a pipe hammer wrench & no one else was holding the clamp. The safety clamp got loose & slipped trapping IP’s right hand between the hammer wrench & auto slips causing a cut on his right ring finger. The IP was initially seen by the Rig Medic then taken to Fahud PDO clinic. He was then transferred to Nizwa Hospital where he was diagnosed as having a fractured right ring finger. • Injury : • Fractured finger • 3 weeks off work.

Back to Main Page LTI Notification LTI No 28 2013 6th May Accident Location: PDO Terminal at Mina Al Fahal WHAT HAPPENED : This is to notify staff of LTI#28 which occurred on the 06th of May 2013 at approximately 0800 hrs, involving a 46 year old Production Operator (IP) working for PDO Terminal at Mina Al Fahal.  Prior to isolation of inlet prover valve at the metering area he stepped on loose grating and fell into a 2metre deep pit landing on his right foot. The IP reported to a private clinic next day morning while on rest days where the X-ray revealed a fracture to his right foot. Six days later he reported to PDO clinic in MAF and he was immediately referred to Khoula Hospital for further evaluation and treatment. The events leading to the incident and the late reporting are currently under investigation. Back to Main Page LTI Notification LTI No 29 2013 4th August Accident Location: Qarn Alam Burhaan, Rig-77 Deutag WHAT HAPPENED : This is to notify staff of LTI#29 which occurred on the 04th of August 2013 at 21:30 hrs involving a 33 year Driller (IP) working for Oman KCA Deutag in Burhaan, Rig-77. After working on a travelling block, the Driller was planning to rig up a stand pipe manifold. He was calling the crane operator to shift the crane, as he was walking sideways towards the staircase opening (forgetting that the staircase had not yet been installed), at the same time signalling to the crane operator. He failed to notice the warning tape which had been put around the staircase area. Inadvertently, he took a step into the opening, and fell from the rig floor to the ground (9m height), landing between the Choke manifold skid & substructure. The IP was initially examined by the Rig Medic and his vital signs were stable. He was then transferred to Nizwa Hospital where he was diagnosed as having suffered a minor head injury with multiple fractures. • Injury : • Multiple fractures • 3 weeks off work.

Back to Main Page LTI Notification LTI No 30 2013 23rd August Accident Location: Nimr G & Karim West Project Galfar WHAT HAPPENED : This is to notify staff of LTI#30 which occurred on the 23rd of August 2013 at approximately 10:30 hrs involving a 35 year old Indian grinder (IP) working for Galfar in Nimr G & Karim West Project. A mechanical crew was preparing to cut a 14” carbon steel pipeline situated inside a pre –padded trench.   The  crew had raised the pipe onto wooden skid supports in order to allow it to be cut.  The IP entered the trench tasked with removing the protective coating on the pipeline by using a chisel but before he had started to work one of the wooden pipe skids tilted and the pipeline rolled off hitting the IP. The IP was able to climb from the trench and walk to the Canter assisted by a co-worker and transported to the Galfar Nimr clinic where he was diagnosed as having tenderness at the hip joints  & lower abdomen. He was not able to sit or mobilize his lower legs.   The medical team immobilized his spine before transferring him to Salalah hospital where he was diagnosed as having a pelvic fracture by CT scan. • Injury : • Fractured pelvic • 6 weeks off work.

Back to Main Page LTI Notification LTI No 31 2013 30th August Accident Location: Saih Nihayda SN-46, Halliburton WHAT HAPPENED : This is to notify staff of LTI#31 which occurred on the 30th of August 2013 at 16:45hrs involving a 43 year old Indian electrician (IP) employed by Innovative Electronics, a sub contractor to Halliburton working on well SN-46 in Saih Nihayda.Following a rig move, the tea room trailer was set up for use however the installation of the stairway to its doorway had not been fully completed as it had not been fixed securely to the trailer. As the IP stepped onto the stairway from the team room they collapsed and he fell to the ground from a height of 1.2m, landing on his left wrist.   The IP was initially seen by the site medic then taken to Qarn Alam clinic. He was then transferred to Nizwa Hospital where he was diagnosed as having a fractured left wrist joint. • Injury : • Fractured wrist joint • 4 weeks off work • Sand Accumulation in the bracket • Stairs resting unsecured against the bracket

Back to Main Page LTI Notification LTI No 32 2013 05th September Accident Location: Rig-83 in Zalzala - Harweel Asset, Sea & Land WHAT HAPPENED : Whilst on the drill floor making a connection on the 5” drill pipe the IP was holding the back-up tong when the tong slipped from the pipe, unlatched and struck the IP’s left arm.   He was immediately taken to the rig clinic for check up and initial treatment after complaining of severe pain and noticing swelling of his arm. The rig medic provided pain relief, splinted the arm and referred him to Salalah Hospital where X-rays identified he had fractured his left arm. He has been transferred to Khoula hospital, Muscat for further treatment. • Injury : • Fractured arm • 6 weeks off work Tong slipped from pipe, unlatched and hit IP on left arm

  31. Back to Main Page LTI Notification 26th September LTI No 33 2013 Accident Location: Nimr, Al Turki beam pump yard. WHAT HAPPENED : In preparation for lifting, the IP had just finished attaching a webbing sling to a beam pump motor guard which had been placed in the corner of the yard in an upright, unstable (unsafe) position.   As the IP turned and walked away from the motor guard it destabilised and fell over striking the rear of the IPs right lower leg, causing a deep cut below the calf muscle. His co-workers and supervisor moved the IP to the Al Turki clinic where the male nurse treated the IP for his visible injuries. After consulting the PDO Doctor the IP was transferred to the Sultan Qaboos hospital, Salalah for further treatment where he was also diagnosed with three fractured bones in his right foot. Motor guard topples over after becoming unstable. • Injury : • Foot fracture • 6 weeks off work Motor guard strikes IPs leg

  32. Back to Main Page LTI Notification 27th September LTI No 34 2013 Accident Location: Rima Shahin field Petrogas. WHAT HAPPENED : After completing their work, the ATOC crew were preparing their equipment to travel to the Aseel field and the IP was securing the wire rotary reel which had been loaded on the back of a lorry. In order to ensure the hose did not unravel during the journey he was inserting an iron bar between the coiled wire and the frame of the reel to act as a stopper.  As he was doing this the rotary reel rotated slightly and trapped his left hand between the iron bar and the frame of the reel.  He was given first aid treatment for a cut at the base of the middle finger by the rig medic and then transferred to Haima hospital where X-rays confirmed a fracture. Back to Main Page LTI Notification 28th October LTI No 35 2013 Accident Location: Fahud C station STST WHAT HAPPENED : The IP with three of his colleagues were manual handling a 6" Sch40 L shaped spool when he tripped over the wooden support of another spool which caused him to lose his balance.  Whilst correcting his balance, the spool he was carrying dropped down and trapped his right index finger between its flange and the other spool. He was initially given first aid then treated for an open wound in the STST Fahud clinic before being transferred to the Nizwa hospital where he was diagnosed as having suffered a fractured finger. Back to Main Page LTI Notification 20th November LTI No 36 2013 Accident Location: Rig 11 in Lekhwair KCA Deutag WHAT HAPPENED : The IP was attempting to bend an allen key in the rigs mechanical workshop and to do so had placed the key in the jaws of a bench vice.  He then started to hammer the key using a steel bar but on the first hit the allen key broke in two and part of the metal flew into the IP’s right eye.  He was immediately provided medical care by the camp medic who then arranged a transfer to Ibri hospital where he was diagnosed as having a corneal tear. He underwent an operation on his eye and is expected to be absent from work for 2 months.    An investigation is underway. Back to Main Page LTI Notification 1st December LTI No 37 2013 Accident Location: MSE, Muscat BMF building WHAT HAPPENED : The IP was working in PDOs Muscat BMF head office as a member of the PDO Corporate HSE Audit Team auditing the company’s IVMS management system.  After the audit team had concluded for the day, the IP was walking along the second floor corridor which was in the process of being cleaned, when he slipped and fell over.   The IP was immediately tended to by his colleagues and then taken by PDO ambulance to Khoula hospital for medical treatment where they diagnosed he had dislodged his knee cap. • Injury : • Dislodged Knee Cap • 3 days

Back to Main Page LTI Notification 7th December LTI No 38 2013 Accident Location: Petrozone , 07/12/2013, Rima WHAT HAPPENED : The driver was travelling from Marmul to Bahja after delivering drill bits to Rig-38 in a three ton truck. Approximately 15km from the Rima roundabout he drove into the back of third party articulated trailer that was travelling in the same direction.   The IP was taken to Nimr PDO clinic by a third party vehicle which stopped to help. He received a check up and initial treatment from the clinic and then he was transferred to Salalah hospital where X-rays identified he had fractured a bone in his face near to his right eye. • Injury : • fractured bone in face near the eye • 5 days

Back to Main Page LTI Notification 14th December LTI No 39 2013 Accident Location: Dalma Energy Rig-46 , 14/12/13, Nimr WHAT HAPPENED : The IP moved the mud bucket that was disconnected from the air winch to lay down a drill pipe pup joint, and then he moved towards the mud bucket and forgotten that it was no longer connected to the winch line. The heavy mud bucket started falling over and he tried to arrest the fall with his hands. There was a 3 ½” drill pipe slips on the rig floor and his left hand got trapped between the mud bucket and the slips, resulting in finger injury. The IP received a check up and initial treatment from the Rig clinic and then he was transferred to Haima hospital where X-rays identified he had fractured his left hand middle and ringer fingers and referred him to Khoula hospital orthopaedic clinic for further treatment. Back to Main Page LTI Notification 4th December LTI No 40 2013 Accident Location: 041213 Saih Rawl Larsen & Toubro WHAT HAPPENED : The IP was involved in photographing the worksite and whilst doing so he began to feel faint.   He decided to sit down and rest on a concrete foundation base but whilst bending to sit, his hand slipped and he fell over landing heavily on his left side. The IP was taken to the Larsen & Toubro site clinic for a check up and after initial treatment he was transferred to the Badr Al Sama clinic in Muscat where X-rays identified he had fractured his left elbow and wrist. • Injury : • Elbow fractured • 3 weeks

Back to Main Page LTI Notification 15th December LTI No 41 2013 Accident Location: PDO- UID, MAF Terminal WHAT HAPPENED : The IP was ascending a metal stairwell at the back of marine office when he slipped and lost his balance.  His left foot hit the stairs and as he attempted to regain his balance the rear of his foot struck against a post supporting the stairwell handrail.  Initially there was no sign of any serious injury and he continued his duty until the early hours of the morning where he found his foot had started to swell, he went to Khoula hospital where an X-ray revealed fractures to his toe and heel. • Injury : • foot injury • 10 days Upright on which he struck his heel

Back to Main Page LTI Notification 21th November LTI No 42 2013 Accident Location: Weatherford, Burooj 21/11/13 Nimr WHAT HAPPENED : At 14:20 Hrs while rig up, Weatherford equipment for RIH 4 ½ Tubing Weatherford Power Tong was hung on winch at rig floor. The tong position was behind the tong post Driller’s Side. Drillers informed the floorman to pickup the power tong and place it near to rotary table using the winch. The Floorman held the power tong on the driller’s side whilst the Weatherford technician held the power tong on the green handle.  As soon as the winch began lifting the power tong, it swung due to its weight. The Weatherford technician hand was trapped between the green handle and rig post. The IP received first aid treatment from the rig medic, (IP was transferred from rig clinic to PDO Marmul clinic) and then transferred to Salalah hospital.  The initial examination of the X-ray showed no fracture but the IP received a slab treatment application and was advised to follow up. On the subsequent follow-up visit three weeks later at Nizwa Hospital X-rays identified a fractured middle & third carpal bone. • Injury : • Hand fracture • 6 weeks

Back to Main Page LTI Notification 21th December LTI No 43 2013 Accident Location: ADS, 21/11213 Fahud WHAT HAPPENED : While busy with POOH 8.5" directional assembly, the crew were removing the rotating head of the BOPs before picking up the BHA. After removing the rotating head, the crew were re-installing the cavin slips. The Hydraulic utility winch was used to lift the cavin slips, whilst the IP was trying to install the required securing pin. The winch wire moved unexpectedly, causing the cavin slips to move. The IPs finger was trapped between the pin and the body of the cavin slips. The IP received first aid treatment from the rig medic, and he was transferred to PDO Fahud clinic, who in turn referred him to Ibri hospital where X-rays identified he had fractured the tip of his right index finger. • Injury : • Finger fracture, • Two weeks

  42. Back to Main Page Fatal LTI Notification LTI No 44 2013 23rd December Accident Location:Qarn Alam Junction into the highway towards Adam direction (Muscat Salalah Road). WHAT HAPPENED:This incident resulted in two fatalities. The deceased were a 24 years old UAE National and 52 years Old Pakistani National Driver transporting materials for PDO project in the South Oman.   The UAE National was travelling towards Adam in a private vehicle and in the process of overtaking he came to a head on collision with a truck that was travelling in the opposite direction. Both drivers of the private vehicle and the truck died on the spot and there were no passengers in any of the involved vehicles.
