Peste Polmonare • Incubazione: 2-3 d • Sintomi • Polmonite fulminante; sputo ematico; shock settico; febbre alta; brividi; cefalea; possibile coagulazione intravascolare disseminata • Diagnosi • Laboratorio: Batterioscopia (Gram) su sangue, escreato, bubboni • Piccoli coccobacilli Gram negativi, poco colorati, bipolari (a spillo da balia)

Peste polmonare • Isolamento! • Fortementecontagiosa • Isolamentorespiratoriostretto, fino a 3 ggdopo la terapia • In seguito, precauzioni standard contro le gocciolinerespiratorie (maschere, camici, guanti, paraocchi) • Trattamento • Streptomicina, doxiciclina, o cloramfenicolo • Alta letalità, ma buonarisposta a trattamentoprecoce

Definizioni epidemiologiche • Possible case • Sudden onset of severe, unexplained febrile respiratory illness • Unexplained death following a short febrile illness • Sepsis with Gram-negative coccobacilli identified from clinical specimens • Probable case • A case that clinically fits the criteria for suspected plague, and in addition, positive results are obtained on one or more specimens • Definitive diagnosis • A clinically compatible case with confirmatory laboratory results • Culture of Y. pestis from a clinical specimen and confirmation of identification by phage lysis • A significant (4-fold) change in antibody titre to F1 antigen in paired serum samples • A presumptive diagnosis, by positive PCR or detection of F1 antigen on suspect isolates, will be available within one working day
