Health Facilitators will work with health care services to support people with a learning disability to access the services they need. • Health Facilitators will also provide training a support to health care staff to understand the needs of people with a learning disability. • The service will be based in the community to promote health and social inclusion.

21. To ensure that individuals health conditions are detected earlier through health screening and the appropriate and prolific use of Health Action Plans. • We will provide a service that links with the Community Adult Learning Disability Teams, the Acute Liaison Nurses and Primary Health Care Services. • We will ensure that the same services are available to people across the county.
