  1. Florida Learn & Serve Instructions for Administering Pre- and Post-Survey and Data Instruments

  2. Reasons/Purposes for Surveys and Data Collection It is required by the Corporation for National and Community Service (CNCS), which funds Florida Learn & Serve. Grant requirements from the Florida Department of Education (included in RFPs for Pilot, Renewal, and District Infrastructure projects) To provide you, your school, your district, the Fla. Dept. of Education, CNCS, and Congress with info about impacts service learning has on student participants.

  3. Instruments and Forms The following items are in this packet: • Parental Consent form template, English and Spanish • Student Assent form template, English and Spanish • Civic Engagement Student Survey (pre) • Civic Engagement Student Survey (post) • Achievement Motivation Survey (middle and high school only) • Students’ Attendance and Behavior Data Summary Form • FCAT Data Summary Form

  4. Timeline/Deadline—Pre-Survey • Give out and get back the consent and assent forms • Administer the pre-survey before service-learning activities begin, or • As soon as possible after activities begin. • Nov. 5, 2010: Return completed pre-surveys

  5. Deadline—Post-Survey, Achievement Motivation Survey (MS and HS only), & Data Forms • Administer post-survey before end of school year • Gather other data at end of school year or in June 2011 • June 30, 2011—all materials due

  6. Pre- and Post-SurveysObtaining Student Assent and Parental Consent • The U.S. Department of Heath and Human Service (HHS) has guidelines for gathering information from human subjects: www.hhs.gov/ohrp/humansubjects/guidance/45cfr46.htm#46.111 • Consult your district testing, research, evaluation, assessment, or grants office • Customize the provided assent and consent templates to fit your district protocols. • Assent/Consent forms are for your records and are not to be provided to FL&S. Student identity is to be kept anonymous and we should not be given the forms which have student names on them.

  7. Active vs. Passive Consent • Check about your district policy • If using active assent and consent, you survey students (1) who turn in assent forms and (2) whose parents who sign and turn in consent forms. • If using passive assent and consent, you survey all students except those (1) who decline to participate or (2) whose parents declare their child should not participate.

  8. Consent Form for Students 18+ Years Old • High school students age 18+ are not required to obtain parental consent. They can provide their own consent to participate.

  9. Other Survey Instructions • Take care of parent consent forms first—this will determine which students then need the assent forms. • Give assent forms to students who will participate both in service learning and in the surveys. • Select students who take both the pre-survey and the post-survey at the end of the school year/service-learning project. • Do not send these forms to us. Again, they would reveal student names.

  10. Administering the Civic Engagement Student Pre- Survey • Pre-assign a Student ID number for each student. Keep a record of ID #s so you can assign the students the same #s for the post-survey. • Allot up to 30 minutes to explain, distribute, and have students complete the pre-survey. • Distribute the pre-survey. • Go over the key info in the assent form that explains the purposes of the survey and students’ options to decline to participate.

  11. Administering the Pre- Survey, continued 5. Do not tell the students this is a pre-survey to be followed by a post-survey later 6. Stress the importance that students be honest and take the questions seriously. 7. Be available if students have questions or concerns. 8. Collect the surveys when students are done. 9. Send the packet of completed surveys by November 5, 2010 to: Joe Follman, Florida Learn & Serve, 820 East Park Avenue, Building F, Tallahassee, FL 32301.

  12. Administering the Civic Engagement Post-Survey • Give at the end of the service-learning project or end of school year. • Assign students the same Student ID they had with the pre-survey. • Students who did not take the pre-survey do not need to take the post-survey. • Repeat steps 2-9 used for the pre-survey. • Send the packet of completed surveys by June 30, 2011 to: Joe Follman, Florida Learn & Serve, 820 East Park Avenue, Building F, Tallahassee, FL 32301.

  13. Instructions for Administering the Achievement Motivation Student Survey (MS & HS Only) • Give at the end of the service-learning project, and with the Civic Engagement post-survey. • The Civic Engagement post-survey ends with question #64; the Achievement Motivation survey begins with question #65. • The surveys will be sent by June 30, 2011 to: Joe Follman, Florida Learn & Serve, 820 East Park Avenue, Building F, Tallahassee, FL 32301.

  14. Completing the Attendance and Behavior Data Summary Form • Form captures summary data comparing total #s of absences, detentions, and suspensions of student service-learning participants with all students in the same grade(s). • These data are already collected by your school/ district—you just need to get and enter the totals. • No individual student information is requested. • Form does not request totals for your entire school—only grade-level comparisons in grades where students are participating in service-learning projects. • Send the completed form by June 30, 2011 to Joe Follman, Florida Learn & Serve, 820 East Park Avenue, Building F, Tallahassee, FL 32301.

  15. Completing the FCAT Results Data Summary Form • Complete all pertinent sections. • Data provide a comparison of 2009-10 and 2010-11 FCAT performance of students participating in FL&S with non-participating students in the same grade, school, and district. • Except for your classroom data, all the other requested data should be readily available from your school district. • Send FCAT results by June 30, 2011 to: Joe Follman, Florida Learn & Serve, 820 East Park Avenue, Building F, Tallahassee, FL 32301. • In case of a delay in the release of FCAT scores notify FL&S immediately.
