  2. State Performance Plan • Not later than 1 year after the date of enactment of IDEA 2004, each State shall have in place a performance plan. • Each State shall establish measurable and rigorous targets for the indicators established under the priority.

  3. Percent of youth aged 16 and above with an IEP that includes coordinated, measurable, annual IEP goals and transition services that will reasonably enable the student to meet the post-secondary goals. Indicator 13: Effective Transition

  4. Changes in Regulations Pertaining to Indicator 13 • Before age 16 • Transition Services (Course of Study) • Coordinated • Post School Outcome Goal • IEP Goals toward Attaining the PSO Goal

  5. Percent of youth who had IEPs, are no longer in secondary school and who have been competitively employed, enrolled in some type of postsecondary school, or both, within one year of leaving high school. Indicator 14: Effective Transition
