He is a smoker with Hx of 10-pack-years but no other significant disease. On exam. he has Pulse 112/min, B.P. 90/60 mmHg, R.R. of 40/min, T 97.6F.Neck veins are engorged and breath sounds are reduced on the left side of chest with a resonant note to percussion. • What is your diagnosis? • How will you manage this patient?

On exam he’s dyspnoeic with R.R of 28/min pulse 98/min B.P 100/79 mmHg with cold moist skin distende neck veins, bilateral basal crepts and scattered ronhi and an S3 gallop.
