  3. FEES • Football - $150 • Soccer, Swim, Volleyball, Tennis, Cross Country, Cheer, Dance - $100 • Girls Club Rugby - $30 • Fee Maximum • $300.00 for Individual • $600.00 for Family

  4. IN ORDER TO PARTICIPATE YOU MUST HAVE… • Current WIAA Athletic Permit Card (Physical Card) MUST have a physical AFTER April 1, 2013 • Athletic Training Services Permission Form-Wheaton Fransiscan Healthcare • Athletic Permission Slip • Concussion Consent Form • Code of Conduct

ACADEMICS • Must be a full time student-enrolled in at least six classes • Minimum G.P.A. of 2.0 (“C” Average) and no more than one failure or incomplete • If you are ineligible from last quarter (2013-14 School Year) When the earliest allowed WIAA game/meet takes place before the first day of classes, or school is out of session (ex. Fall Sports), the maximum ineligibility period shall be the lesser of: 21 consecutive calendar days, beginning with the date of the earliest allowed competition in a sport or 1/3 of the maximum number of games/meets allowed in a sport (rounded UP if 1/3 results in a fraction). In this instance, a scrimmage is not considered a “game” and multi - game tournaments only count as “1” game

  7. ATTENDANCE • Student-athletes must be in attendance for the FULL school day in order to practice or compete that evening. • Prior approval must be made with an administrator for excused absences such as funerals, college visits, MD/Dental/Eye appointments or other family circumstances. • A suspension (in or out of school) student may not participate during the suspension period.

  8. WHERE ARE THE FORMS?

  9. TRANSPORTATION • All athletes are expected to travel to and from away contests with their team. • Only students with written permission by parents and signed by athletic director prior to the contest will be allowed to leave with their parents from a contest and not travel back with the team.

Impact Testing • Bethany Shaw, ATC/LAT • Neuro-cognitive test (memory, reaction speed etc) • One measure used to determine if ready to return • Baseline required prior to start of season • 20-30 min testing time • Conducted here at WHS • NO COST TO FAMILIES! • ImPact testing is REQUIRED prior to first day of tryouts • Testing time Schedule to follow

  13. COMMUNICATION IS KEY Understand Chain of Command • Athlete-coach communication • Athlete/parent-coach communication • AD-athlete/parent-coach communication • Principal-AD-athlete/parent

  14. BOOSTER CLUB • Meets on the second Thursday of each month in the library 7:00 pm • Provides financial support to ALL school activities 70% to Clubs/Athletic • Raises funds through memberships, events, school concession stand, auction and more • Sells event passes and Whitnall Falcon wear

  15. SOME REMINDERS • Sportsmanship • Treat officials, opponents, bus drivers, and school personnel with respect. • The Athletic Code is in effect year round and for your entire high school career.
