  3. What is the Fall Management Program? The Fall Management Program is a preventative strategy that employs interventions to prevent falls. Upon admission, each client is assessed using the Fall Risk Assessment Tool. If the client is identified as a fall risk, the Fall Management Program is initiated.

  4. Program Goals • To create a safe environment. • To identify patients at risks. • Initiate preventative measures. • To involve participation of clients, their families and stakeholders.
