  2. Purpose • The Silver Bridge connected Point Pleasant, West Virginia and Kanauga, Ohio; and was made with chained carbon steel “eye-bars” and wire cables.

  3. How It Failed • Due to stress corrosion, one of the heat treated eye-bars broke, causing tension on the surrounding eye-bars. And the traffic of cars on the bridge increased the weight of the bridge adding to the cause of the collapse on December 15, 1967.

  4. Corrective Actions • At the time of constructing the bridge, they used steel not known for resistance against corrosion on the steel. They could have used a steel that was proven to be resistive.

  5. Final Actions Taken • After the bridge had completely collapsed, there was an investigation to find out why it had fallen and the government mandated new inspection policies for better maintenance of bridges. National Bridge Inspection Standards (NBIS)
