  • Medical, Dental and Eye exams are encouraged (routine check-ups)

  • Forms included in packets

Health Alerts

  • Health history (obtained from parent/physician input)

    updated yearly and “as needed” or requested

  • Shared with school staff on a “need to know” basis

  • Determine if it is a Health alert or IHP (individualized health plan)

  • IHP developed between school nurse and parent

    • Indicates health condition/illness

    • Specifies school implications and or emergency measures needed

    • Identifies parent contact information (important to keep current)

  • Medications must be brought to school in original labeled container with student name

  • Medication authorizations

    • Prescription medication require parent and provider signature

    • Over the counter medications require parent signature

    • Forms available in school office and district website

    • Wisconsin immunization Registry (WIR)

    • Communicable Disease Center (CDC)
