  • Determine if it is a Health alert or IHP (individualized health plan)
  • IHP developed between school nurse and parent
    • Indicates health condition/illness
    • Specifies school implications and or emergency measures needed
    • Identifies parent contact information (important to keep current)

    • Prescription medication require parent and provider signature
