The Future • As of August 2018, 1,974 over 65’s were included on a GP dementia register in Trafford. • People are living longer with more complex conditions. • The estimated dementia diagnosis rate among over 65’s in Trafford (74.5%) is above the (66.7%) target and higher than the England average (67.5%). • Between 2016 and 2031 the number of people aged 65+ in Trafford is projected to grow by 11,700 (29%) (ONS, 2016-based subnational population projections)

  4. So what is it? • The idea is that the Adopt a Care Home scheme will support our local care homes to link with local schools. The Adopt a Care Home scheme aims to: • Contribute to teaching school children about dementia and our elderly community • Introduce some children to elderly people • Link them with people living with dementia in a local care home • Increase understanding for children • Increase engagement and social inclusion for the residents

What are the benefits? • Introduce children to older people they might not otherwise come into contact with • Increase the Childrens knowledge and understanding of dementia awareness • Introduce them to the idea of health and social care as a career aspiration • Older people will benefit from increased social inclusion and improved health and wellbeing as a result of making new friendships with young people in their community • Increased social inclusion and increased appetites and communication in the homes - Social value • Promotes reminisence
