1. Central Registration Frequently Asked Questions Q. When will central registration begin for the 2007-08 school year? Our central registration office will open on March 17, 2007 for all new students in kindergarten through 12th grade. Registration into the Paulding County School District is a two-step process: Step 1 – central Registration – Parents visit our central registration office and complete forms to be admitted to the district. Step 2 – Enrollment – Parents visit the assigned school to obtain class assignments, schedules and other information unique to that school.

2. Central Registration Frequently Asked Questions Q. When can I enroll my child for kindergarten for the 2008-09 school year? Kindergarten registration for the 2008-09 school year will be held at the district’s elementary schools on March 12, 2007, from 9 a.m.-2 p.m., and March 13, 2007, from 9 a.m.-2 p.m. and 6 p.m.-8 p.m. If you miss these dates, please contact the school for an appointment. After March 17, 2007, kindergarten students registering for the 2008-09 school year will need to register at the central registration office.

3. Central Registration Frequently Asked Questions Q. Will all parents need to visit the central registration office to enroll their student into a school? No, only parents with students new to the district need to visit the central registration office. Parents who have changed their address need to visit their child’s assigned school in order to verify their new address; no other registration forms will be required.

4. Central Registration Frequently Asked Questions Q. Where do I go to register my child for school? Do I need an appointment? For students new to the district, registration in the Paulding County School District is a simple two-step process. The first step is to visit our central registration office. Our central registration office is located at 522 Hardee Street, Dallas, Georgia, 30157. Please call to schedule an appointment. The hours of operation are 7:30 a.m.-11:30 a.m. and 1:00 p.m.-4:00 p.m. Monday-Friday. Walk-ins will be seen as time becomes available between scheduled appointments. Please see the Required Documents Checklist for a list of documents to bring. After you complete the registration process, you and your child will visit the assigned school to obtain class assignments, schedules and other information. Please note that the registration process is not finished until you complete the enrollment process at your child’s assigned school.

5. Central Registration Frequently Asked Questions Q. What is needed to verify residency in the Paulding County School District? In order for a student to be admitted in the district, the person enrolling the student shall provide proof of residency within the district. Proof of residency shall be provided to central registration personnel when the student seeks initial enrollment in the district.

6. Central Registration Frequently Asked Questions Q. What other information is needed to register my child for school? All parents/guardians will be required to show a picture I.D. and provide proof of residency (see above). You will also need to provide your child’s birth certificate or other official documentation, immunization records (Immunization DHR Form 3231 and Hearing, Vision, and Dental Examinations DHR Form 3300), and previous school information. Again, please see the Required Documents Checklist for a complete list of documents to bring. Students must be 5-years old on or before September 1 to register for kindergarten, and 6-years old on or before September 1 to register for first grade. In addition, please bring the names and phone numbers of those individuals you will designate as emergency contacts.

7. Central Registration Frequently Asked Questions Q. Where can I go to obtain the immunization records required for my child? Immunization records may be obtained through your health care provider, the clinic where your child received vaccinations, or from the previous school your child attended. Students attending a Georgia public school must have their immunizations documented on a Georgia DHR Form 3231. Likewise, students enrolling in a Georgia public school for the first time must have hearing, vision, and dental screenings documented on a Georgia DHR Form 3300. If a student is transferring from outside of the state of Georgia, all out-of-state immunizations must be transferred to a Georgia DHR Form 3231. The Paulding County Health Department will transfer your child’s immunizations to a Georgia DHR Form 3231. The Paulding County Health Department is located at 451 Jimmy Campbell Parkway, Dallas, Georgia, 31032, and their phone number is (770) 443-7881.

8. Central Registration Frequently Asked Questions Q. If I have more than one child to register in the district, may I register all of my children at the same time? Yes, parents may register all of their students, kindergarten through 12th grade, at our central registration office during step 1 of our registration process.

9. Central Registration Frequently Asked Questions Q. Do students need to accompany their parents to the registration process? Students do not need to accompany parents to our central registration office to complete step 1 of our registration process; they must accompany parents in step 2 when they visit schools to enroll.

10. Central Registration Frequently Asked Questions Q. How long does it take to complete the registration process? District registration forms take approximately 10-15 minutes to complete for each child. These forms are available at the central registration office and on our web site at http://www.paulding.k12.ga.us/. Once all forms have been completed, parents will meet briefly with a central registration staff member to answer questions, review forms, and verify residency. Actual time to complete the registration process will vary upon the time of the year.

11. Central Registration Frequently Asked Questions Q. May I complete the registration forms before going to the central registration office? Yes, parents are encouraged to complete forms prior to visiting our central registration office. These forms are available at the central registration office and on our web site at http://www.paulding.k12.ga.us/.

12. Central Registration Frequently Asked Questions Q. If I move from one address to another within Paulding County, how do I enroll my child at the new school? Does my child have to change schools? Any time a family changes their address, they must visit your assigned school to verify residency. If the new address is still within the Paulding County School District, no additional forms will need to be completed. Once the address is verified, parents will be notified if there will be a change of school.

13. Central Registration Frequently Asked Questions Q. If I am buying or renting a new home in the district, may I enroll my child before I move? Yes, parents will need to provide a rental agreement or a purchase contract with a possession date (not the closing date) listed on the contract. Parents will need to complete the 60-Day Enrollment Form.

14. Central Registration Frequently Asked Questions Q. What are the requirements for registering my child into kindergarten? All parents/guardians will be required to show a picture I.D. and provide proof of residency prior to registering a student. A birth certificate or other official documentation and immunization records (Immunization DHR Form 3231 and Hearing, Vision, and Dental Examinations DHR Form 3300) are required for all students. Students must be 5-years old on or before September 1 to register for kindergarten.

15. Central Registration Frequently Asked Questions Q. How do I enroll my child in a specialized program, such as gifted services or special education services? You will indicate your child’s participation in any such programs when you complete the required paperwork at the central registration office. Actual placement into specialized programs will be discussed at the assigned school during the enrollment process.

16. Central Registration Frequently Asked Questions Q. Once I complete the central registration process, how do I find out more about my child’s assigned school? The central registration office will provide you with an information sheet on the school your child will attend. This sheet will include a school map and location, hours of operation, a brief description of the school, and “next steps” to enroll at the school. In addition, information for all of our schools is provided on our web site at http://www.paulding.k12.ga.us/.
