  2. Veteran Eligibility • Must complete a VA form 10-10EZ to determine financial eligibility • Submit the 10-10EZ with the Veteran’s DD-214 (will place Veteran in 1 of 9 priority categories) 1-800-827-1000 www.benefits.va.gov/benefits/ • Make an appointment with a VA provider

  3. Non Institutional Care Programs • Adult Day Health Care (ADHC) • Homemaker/Home Health Aide (H/HHA) • Respite services (Inpatient and home) • Purchased Skilled Home Care (PSHC) • Home Hospice • Home telehealth • Medical Foster Home (MFH) • Veteran Directed Home Community Based Services (VD-HCBS) • Bowel and bladder • Home Based Primary Care (HBPC) • Care Giver Support Program (CGS)

  4. Adult Day Health Care • VA purchased through community vendors • VA authorizes a specific number of days (1-2) for Veteran to receive services in the community while a CG gets respite or works • Usually used for cognitively impaired Veterans • May carry a $15/day co-pay • Requires an application for extended care services (VA form 10-10EC) • Best used for Veterans with cognitive issues

  5. Homemaker/ HHA • Provider determines need, places a consult for assessment of need level • A licensed staff member performs an assessment of ADL/IADL needs • Clinical review performed and hours are awarded • Awarded based on a case mix budget tool

  6. Respite Services • Institutional respite for 24 hour care • In-home services will cover up to 6 hours • All registered Veterans have 30 days each 12 month period • Requires an application for extended care services (VA form 10-10EC) • May carry a $15.00/day co-pay for home respite • May carry a $89.00/day co-pay for institution respite

  7. Purchased Skilled Home Care • Used many times as transition care from an acute hospital to home • Skilled services, nursing, PT, OT, ST • Could be ordered after multiple falls in the home • Transition care from SNF to home

  8. Home Hospice • VA purchased through authorized community vendors • Helps transition families from a state of health to end of life care • All Registered Veterans are eligible

  9. Home Telehealth • Monitor certain chronic illnesses • Congestive Heart Failure • Hypertension • Diabetes • Alerts Providers when an intervention is needed eliminating a hospital admission • Allows geographically distant Veterans to have more engagement with their providers

  10. Medical Foster Home • A residential home that provides room, board, and personal care • Community families invite the Veteran into their homes after being approved • Monthly fee involved ($1,900-$2,700/month depending on level of services needed) Many apply for Aid and Attendance • Used when a Veteran is still somewhat independent but cannot live alone

  11. VD-HCBS • Partnership with County agency for aging (Philadelphia Corporation on Aging) • Similar referral process as for H/HHA • Veteran is awarded a monthly stipend to hire their own aides (paid through a fiduciary) • Helpful for cognitively impaired Veterans who can’t tolerate strangers, younger SCI or TBI Veterans

  12. Bowel and Bladder • Mainly for our SCI Veterans • Hire a family member to help with an elimination schedule • The aide is trained by the VA • Gets paid as a GS 5

  13. Home Based Primary Care • Homebound or great difficulty to leave home • All disciplines make house calls (provider, nurse, SW, therapies) • Comprehensive care for chronically ill Veterans needing more frequent help • Extra support, coumadin blood draws, frequent hospitalizations

  14. CGS • Unique program that offers support to CG caring for challenging illnesses • Offer conferences to help with estate planning • Social outings for the CG • Monthly conference calls to educate, support • Offers alternative ways to deal with the burden of caring for a chronically ill loved one
