  3. Professional Background • Worked in Mid Wales hospital from 1987 to 1993 • Qualified RMN in 1990 • Brecon Hospital from 1993 to 2008 • CPN for older adults from 2008 to ? • Have always worked with older adults

  4. Project Background • Facilitate a monthly dementia carer’s group which includes: • Talks from local services (health, social care, voluntary services and carers) • Question and answer sessions with these service providers, led by carers • Informal discussions amongst the carers sharing experiences

Project Background • Common theme was lack of information with regard to support available to person and families. • Unaware of professional and voluntary support • Unaware of financial support • Crisis resolution/ Carer breakdown • Family support • Prognosis and ongoing management • Unaware of local and accessible services

  6. Aim • Provide a concise information leaflet detailing local services appropriate for client group to promote: • H A P P I N E S S

Aim • Home care for as long as possible • Awareness of the available support and information • Personal independence, safety and wellbeing at home • Person centred care • Individualised care • Numbers of hospital admissions being reduced • Equality of service • Service provision appropriate for the individual/family • Service provision available to prevent crisis

  8. Objectives • To achieve this aim the carer’s group discussed the need for: • A list of services appropriate to the person with dementia • Relevant information on each service ie. Contact details, brief description of support available and source of referral • A list of local services that can be easily accessed

  9. Methodology • What was the process? • Discussed with line manager • Carer’s and colleagues provided a list of relevant services • Contact made with each service to provide details • Leaflet designed and re-designed

Methodology • Audited via brief questionnaire distributed to carers (Feedback positive !) • Distribution approved by line manager and directorate manager • Liaison with GP surgeries, psychiatrists, hospitals, voluntary and social care agencies for distribution • 100 leaflets printed initially • Amendments made and leaflet reprinted

  11. Service Provision Outcome • Increased awareness and uptake of local services to support the person with dementia to live at home as safely and independently as possible

  12. Personal Outcomes I am now more aware of: • the personal impact of dementia on family members • the voluntary and professional support available • the benefits of networking with other agencies • the need to be more proactive in my role
