What ? • Money • To support a robust package of support at level 1/ 2 using CAF for assessment planning and review • To enable children and young people with disabilities to access universal services and short breaks such as: - Leisure activities - After school clubs - Holiday activities - Workshops, horse riding etc • For 12 weeks plus an additional 12 if required and agreed at TAC/TAF review

  3. What is it not? • To replace statutory provision • To give level 3 services a “top up” • For profoundly disabled or neuro- typical children and young people • To provide specialist services • To create a dependency at any level

  4. To resource and support CAF when working with children with disabilities To prevent crisis in families of disabled children resulting in referral to level 3 intervention To involve the family in the service plan and promote inclusion across universal services Why?

  5. In receipt of Disability Living Allowance (DLA) Has a medical diagnosis Attends a Mainstream or Special School with a diagnosis 0 – 19 Who is it for?

  6. A need is identified in a C/YP who meet the criteria of the access fund CAF is authored, recorded and an Access Fund (AF)form is attached with the cost of the service where applicable (can be found on the intranet under CAF and Aiming High websites on the A-Z) Copy of CAF and AF form received from Lead Professional Admin check level of case on ICS and CAF databases Applications considered at monthly panel meetings Admin inform Lead Professional confirming approval of grant and give details of where to send invoice. or Jenny contacts Lead Professional with explanation of why application has been refused or guidance on how to make it more successful.
