  2. Case Segmentation “Case Segmentation is the practice of dividing up cases into groups that are similar”

Background Merced County began to utilize the Case Segmentation values in CMT in April 2011 Prior to using case segmentation, CMT was used to count case worker production (i.e., number of case reviews completed, contempt actions, etc.) Analysis completed on Post caseload to determine if segmentation beneficial

  5. Case Segmentation Implementation • Phase I • Initial segmentation of Post Team caseloads • Phase II • Monthly case segmentation update

Case Segmentation Implementation • Phase I • Introduction to case segmentation in CMT • Overview of completed caseload • Tips for completing case segmentation • Hands-on training • Initial caseload segmentation was not completed in training – case workers given 1 month to complete on their own.

Case Segmentation Implementation • Phase II • Segmentation completed on Post team caseloads • Post team brought back to training after CMT monthly download • Train staff to sort and manipulate data to quickly identify cases that need action or review • Introduced case management priorities using CMT
