She points to her PIPs and MCPs and wrists. She informs that the stiffness lasts for around 2 hrs every morning and symptoms improve as the day progresses. She denies back pain/ stiffness. She has fatigue and low grade fever, Swollen and reddened MCPs and PIPs but DIPs wnl. Fluid +t in wrist joints. Other examinations are wnl • What is your probable diagnosis? • Give 5 differential diagnosis • How will you approach to manage the case? (10) • What is gout? What are the differences between gout and pseudogout? Write down the clinical features and management of Gouty arthritis. (10) • A 64 year old man comes to you for evaluation of knee pain. He tells you that he had right knee pain for many years but recently it’s worsened. He has no early morning stiffness. On examination, you can hear crepitations as you move his right knee, other examinations are wnl. Lab testing is unremarkable. • What is your probable diagnosis? • How will you approach to manage the case?
