  2. DISCHARGE LIAISON The Forum for Specialist Practitioners in Wales What to Expect? This forum is built on a workshop held in June 2007, which reviewed the role of Discharge Liaison Nurses in Wales. At the workshop participants agreed a number of key recommendations for improving the effectiveness of discharge practices. These were highlighted in the NLIAH report entitled “Lost in Translation” published in November 2007. One of the recommendations that has received widespread agreement and a key to successful improvement, was the formation of a national forum for practitioners who specialise in discharge planning. The forum will be facilitated by the NLIAH Change Agent Team. Their role will be to help and support this community of specialists to increase their professional influence and identify opportunities to become a catalyst for change in their own communities. This national health and social care forum is designed for managers and practitioners who lead on discharge planning, or who work closely with the day to day challenges of getting people home.
