    2. Executive Summary Write this last! Briefly summarize the scope of your project. Illustrate your community’s need. List your objectives and the outcomes to be gained. State the total cost of the project. No more than 4 double-spaced, 12-point font pages. 2

    3. Project Narrative Detailed description of the purpose, goals, objectives, strategies, design and management of the program. Describes the approach for the EUDL program in his/her community. Must be double-spaced, 12-point font, and contain the following components: Project Description (Sections a –f must be 24 pages or less) 3

    4. Project Description Components Needs Assessment Project Goals Project Objectives Implementation Plan Evaluation Future Funding/Sustainability 4

Needs Assessment Must Include the Following: Service area; Number of youth to be served in the service area; Number of alcohol licenses (broken down by bars/alcohol establishments, restaurants, liquor stores, grocery stores, and beer distributors); The approximate number of MIP citations issued and convictions in the last year for the service area; Youth-specific data from the Prevention Needs Assessment or the Youth Risk Behavior Survey for the community (not statewide). 5

    6. b. Project Goals Provide a broad statement, written in general terms, that conveys the program’s intent to change, reduce, or eliminate the problem described. Outline the specific goal(s) of the project. “Our agency will improve the enforcement of underage drinking laws in our community.” 6

    7. c. Project Objectives Identify the specific milestones aimed at achieving the goal(s). State the date when a milestone will be reached. The objective must be specific, measurable, achievable, realistic, and tangible. “Compliance rates for our community will increase to 90% by June 30, 2010.” 7

    8. d. Implementation Plan Describe specific steps that will be taken or projects that will be funded to accomplish each objective. One format is a table that lists the objectives, the responsible party, the timeline, how each objective will be accomplished, projected costs, and resources needed. Attach letters of support and MOUs if other agencies are involved. 8

    10. e. Evaluation Document the data you intend to collect to verify that the objectives have been met. Clearly state what data will be collected, how, by whom, and when. Describe the existing community-level baseline data. Describe the capacity to support the program. Provide assurance of the agency’s commitment to provide pre- and post-data related to underage drinking. 10

    11. f. Future Funding/Sustainability Plan Include a plan that outlines how the project will be funded in the future. Address the current year funding plan. Estimate funding requests for the subsequent year. Estimate the total length of federal funding you anticipate seeking. Provide a general description of funding in the final year of support. A sustainability plan is more than just the intent to apply for more funding! 11

    12. Budget and Budget Narrative Budget must be complete, allowable, and cost-effective in relation to proposed activities. Budget must show the cost calculations to demonstrate how the applicant arrived at the amount requested. Budget narrative is a brief description that links costs with project activities. Budget narrative should demonstrate that all costs are reasonable. 12

    13. Budget Detail Worksheet The budget detail worksheet itemizes the costs and has a short narrative which explains and justifies each budget item. Following is a sample of the Personnel section of a budget detail worksheet: A. Personnel. List each position by title and name of employee, if available. Show the annual salary rate and the percentage of time to be devoted to the project. Compensation paid for employees engaged in grant activities must be consistent with that paid for similar work within the applicant organization. Name/Position Computation Cost Note: Personnel costs are only allowable for direct management and administration of the grant award, i.e., preparation of mandatory post-award reports.     14. Special Assurances and Conditions This is a list of Special Assurances and Conditions that are part of the contract between MBCC and the subgrantee upon award. The signature page binds the applicant to the Special Assurances and Conditions. One example of a Special Assurances and Conditions page is the following: Assurances of Compliance with Civil Rights Act of 1964: The applicant hereby agrees that it will comply with Title VI of the Civil Rights Act of 1964, as amended, and all requirements imposed by or pursuant to Regulations of the Department of Justice (28 CFR Part 42) issued pursuant to that title, to the end that no person shall on the ground of race, color, religion, national origin, sex, or disability be excluded from participation in, be denied the benefits of, or be subjected to discrimination under, or denied employment in connection with any program or activity funded in whole or in part with funds made available under this title. 14

    15. All Successful Applicants Must Agree to the Following Conditions: Submit quarterly narrative, data, and financial reports in the prescribed format according to MBCC time frames. Submit semi-annual reports to the Office of Juvenile Justice and Delinquency Prevention using the Data Reporting Tool (DCTAT). *It is strongly recommended that applicants or project personnel attend one of the MBCC grant management workshops. *Applicants must include travel for a minimum of one program person and one youth to attend training at the National Leadership Conference at an approximate cost of $1,500 per person. 15

    16. Signature Page Original Signatures are required! The signature page binds the applicant to the contract. Duplicate responsibilities are not allowed; no single person can sign as Agency Representative and Project Director. The official budget representative must be a person with budget-setting authority, generally a mayor, chairperson, department director, etc. 16

    18. What is CCR? CCR is intended for the registration of business and organizations only. Anyone (sole proprietors, corporations, partnerships, and governmental organizations) desiring to do business with the federal government must register in CCR. Annual renewal is required to keep your CCR current and make sure you are eligible to receive federal funds. The website is www.ccr.gov. 18
