  3. Main Home page Select "View" next to the instructors name to access forms

  4. Teacher Folder Page This page shows all of the forms for a career teacher: Observation Form PDP Push-Pins and Evaluation Form To complete a form, click GO TO FORM To schedule a conference, click Schedule

  5. Teacher Evaluation Form Complete the Form with the instructors information, use their state issued ODCTE, PID number for their Employee Number

  6. Using the rubric for each indicator Rank the 20 indicators between 1-5, utilize the ? mark symbol to hover over a specific area to reveal the rubric as shown in the illustration

  7. Final Ranking and Comment Section Remember each Domain is weighed differently, the final score is calculated when the form is saved. If a teacher scores 2.7 and below the must be placed on a PDP, Professional Development Plan form. Comments are typed in this section. If an individual receives a ranking of 1,2,4, or 5 the evaluator is required to provide documentation in this section. Rankings of 1-2 require the evaluator to complete a PDP, Professional Development Plan form in addition to this document.

  8. Sharing the File with the Teacher Step One: Form Share prior to conference After you finish writing your comments, you can Form Share the document allowing the teacher to view the document before your conference. Just click Form Share and "read only access" Step Two: Save and Submit - after the face-to-face conference After you hold the conference with the teacher, you are ready to save and submit the form. Click Save & Submit. The teacher will receive an email asking them to electronically sign the form. The evaluator will be notified to sign the form once the teacher signs.

  9. Electronic Signatures After you click Review, you will open up the teachers evaluation form. Scroll to the very bottom and type your name the Signature Box area. You will also have the opportunity to add comments or respond to any comments the teacher may have made while signing their form.
