  2. Winyan kin zaniya unpi kteWomen will live in good health • The women’s health care unit provides many different health screenings for the American Indian women who live in the Aberdeen Area IHS.

  3. The MDT unit is available to women who are 18 years and older and enrolled in the Aberdeen Area Indian Health Service. They will receive the following services: >Mammography (40 yrs. +) >Clinical Breast exam >Pap Smears >Pelvic Exam >Bone Density test >Diabetes Screening >Cholesterol Screening >Health Risk Assessment

Identify Tribal programs that serve eligible women Arrange presentations at these venues at least 1-2 mos. prior to screening Identify local teams for each site: IHS & Tribal programs Disseminate flyers, PSA’s, & Media coverage Develop a list of Community Resources Address & assist with trouble-shooting if challenges occur Provide minimal information on the tests, diseases, prevention, treatment, and follow-up care for the clients Must be on site 2 days prior and 1 day after the screening Develop a standardized tracking tool for each site Document field notes of site experience (each site will vary) Participate in patient services evaluations Assist local tribal programs as they get transportation, scheduling, follow-ups, etc. Coordinate “Thank You’ to clients -1 week after screening Develop tracking system for project materials Community Outreach Activities

  5. Community Outreach Coordinator will contact the Aberdeen Area: • Tribal Presidents • Service Unit Directors • Tribal Health Directors • CHR Directors The purpose of the contact will be for introductory information of the MDT unit prior to it’s arrival and Face to Face meetings with the respected key people for the local teams.

  6. Community Outreach is coordinating activities with State CCC programs and Women’s Health groups within the Aberdeen Area and the 4 state region; (e.g. data information, statistics, billing of services, cancer prevention activities, and cancer plans)

  7. Local Teams should include but certainly not limited to: • Tribal Presidents • Service Unit Directors • Tribal Health Directors • CHR Directors • PHN • Providers (MD, CNM, CNP, PA) • Health Educators • Community Health Representatives • Lab Personnel • Radiology Dept. • State & Tribal BCCP
