  8. Participation Worksheets CEO PARTICIPATION DISTRICT/SPONSOR SITES The District-wide sheet should be used only for the SFAs choosing to do CEO for all of the schools in the district. SFA selecting District-wide CEO will use one set of claiming percentages for all of their sites. SFAs should use the individual schools sheet or the group sheet if not all schools in the SFA are participating in CEO. revised 5/22/2013

  9. CEO PARTICIPATION GROUP SITES When participating by group(s) there must be at least two school sites per group and there is no maximum number of schools that can be a group. However, each site can only be in one group. Each group will have a claiming percentage that will be used for all sites in that group. You may add more groups.If you have some individual sites and some groups, both the Individual School Sites form and the Group Sites form will need to be completed and submitted to Georgia Department of Education (GaDOE). revised 5/22/2013

  10. CEO PARTICIPATION INDIVIDUAL SCHOOL SITES When participating by individual site, each site selected to participate in CEO will have its own claiming percentages. There is also the option of having some schools claimed as individual sites as well as having some schools claimed as a group in order to increase their claiming percentages. If you have both individual sites and groups, both the Individual School Sites form and the Group Sites form will need to be completed and submitted to Georgia Department of Education (GaDOE).

Fee Waivers for High School Students • Parents: • The ACT (American College Testing) and The College Board (SAT, PSAT) offer fee waivers to reduce the cost of the ACT, SAT, and PSAT exams for students from families who qualify under the criteria listed below. Qualifying students should speak with their counselor about fee waivers for these examinations. • The College Board also offers a reduced fee exam for students taking the Advanced Placement (AP) exams each May. The criteria for the reduced exam fee are the same as the criteria for fee waivers (listed below). Qualifying students who plan to take AP exams should speak with their counselor and the AP Testing Coordinator for their school to ensure they are listed on the schools’ records as qualifying for the exam at a reduced fee. • To be eligible, you must meet at least one of the requirements below: • Annual family income falls within the Income Eligibility Guidelines set by the USDA Food and Nutrition Service. • Enrollment in a federal, state, or local program that aids students from low-income families (e.g. Federal TRIO programs such as Upward Bound). • Family receives public assistance. • Lives in federally subsidized public housing, a foster home or is homeless. • A ward of the state or an orphan. • Check all that apply • ______ My household receives TANF or SNAP or other public assistance. • _______My household’s monthly total income is $__________.
