Why develop a Pathway??? • 100 people who have a Learning Disability per year attend/are admitted to the hospital • We know that some people have excellent care • We know that some people’s experiences are not as good • Pathway is designed to set standards for care and to tell us whether we achieve this so that information about our current practice can inform our future practice.

  3. What does the Pathway look like? • Sample/s available for you to look through • Do not be alarmed by the volume of paper!!! • Some sections are relevant to every area • Some sections are specific to some departments/wards

  4. Colour coding • Pages are colour coded for ease of use • Core papers and Appendices : PINK (principles to be used for all patients) • Outpatients: BRIGHT YELLOW • Pre Op Assessment: GREEN • Admissions Ward: BLUE • Theatres: ORANGE • Emergency Admissions/A&E: PURPLE • Discharge Planning section: PALE YELLOW

What does the Pathway consist of? • Text/information to support best practice for people with a Learning Disability admitted/attending hospital • Flow chart to illustrate best practice principles • “Bill of Rights” • Dependency Assessment scale • Traffic light/Hospital Assessment tool • “Easy-Read” letter • Variance record sheet to monitor against standards • Pathway should be use in conjunction with the LEARNING DISABILITY RESOUCE PACK

  6. What is the plan? • Pilot plan to trial with people attending for planned surgery who are known to the Community Learning Disabilities Team • Confined pilot to start • Teaching and information sessions for all areas • Evaluation of the pilot and feedback to pilot areas/those involved • Future roll out of other sections of the Pathway • Links with LEARNING DISABILITY RESOURCE FILES
