  2. Intrahepatic cholestasis of pregnancy ICP • 特点：瘙痒、黄疸、胆酸和肝酶升高 • 病因：不清、雌孕激素代谢、遗传、环境因素 • 对母亲的影响：产后出血、糖脂代谢紊乱 • 对围产儿的影响：早产、FGR、胎儿窘迫和死亡、新生儿颅内出血、神经系统后遗症

  3. ICP的诊断 • 病史 • 临床表现：瘙痒 • 体征：搔抓痕、黄疸、尿色深、肝肿大、胎儿窘迫 • 辅助检查：血胆酸测定、肝功能异常、 • 病理检查：胎盘、肝脏活检 • 鉴别诊断：妊娠期急性脂肪肝、子痫前期、病毒性肝炎

  4. ICP的处理 • 保肝利胆，缓解症状：休息、葡萄糖、维生素、吸氧 • 利胆止痒：考来烯胺、苯巴比妥、熊去氧胆酸、地塞米松 • 加强孕期监护 • 适时终止妊娠 • 预防产后出血 • 不用口服避孕药
