What We Provide: “One Stop Shopping Services” • Counseling, & Testing Services: Rapid Testing • Mental Health Evaluation & Treatment • Substance Abuse Evaluation and Referral to Treatment • Medical Case Management Services • Primary Care & Infectious Diseases Evaluation and Treatment • Nursing Care Management, Health Screening and Discharge Instructions • Risk/Harm Reduction and Counseling Services • Nutrition Counseling • Dermatology Services • Ophthalmology Services • Gynecological Services • Pain Management • Phlebotomy onsite • Transportation Services • Dental Referral

Cervical Cancer Screening Initiative • Problem • Since August, 2012 no longer had a clinic based gynecologist • Realized afternoon GYN clinic not effective for screening • Incentives (gift bags, Shop Rite gift cards) did not improve stats • Had a high % of abnormal PAPs requiring further intervention, ie., colposcopies. • 134 PAPs were done from July 2012 through March 2013. Twenty-five percent (25%) of those PAPs were abnormal. • 117 of those PAPs were screened for HPV. Thirty one percent (31%) of those screened were positive for HPV. • Interventions to improve screenings • Hired PD APN to perform PAPs during busy morning clinics so patient could get PAP on the same day of routine medical visit • Linked with hospital GYN clinic for additional availability for pap screening and routine days for colposcopies • RNs/MCMs increased efforts to retrieve pap smear results performed in outside clinics • APN monitors paps done via an excel spread sheet • Use of huddle teams to review charts and address primary health care needs before patient is seen by provider so that referral to the GYN APN is made on a timely basis • Ongoing chart audits done by RN’s 2-3X/year

Methods of Monitoring • Goal to achieve = 60% females will have annual pap smear done • Report data bimonthly to the NJ Cross Part Collaborative • # females with documented pap smear result in the measurement year/# females > 18 with one medical vs with a provider in the measurement year • Cervical cancer screening is a HAB HIV Core Clinical performance measure, cervical cancer is the most common AIDS related malignancy in women

  5. Results of our initiative • Improvement is slow but….. WE ARE ON AN UPWARD SWING!! GOAL: 60%
