  4. Part One- How/What/Why (Description) • Succinct Description: • Partners/Sites • Member Types/Terms • Activities • Outcomes Most Important Facts That Would Interest/Inform Everyone

  5. Part Two- Your Team • Team Roster • Everyone that has a role in the success of this program should be a part of your team Management Teams need to stay in contact, meet regularly, assess progress

  6. Part Three-Communication and Oversight • Keeping Stakeholders Informed • Management Processes • Training Assessment Processes • Financial Oversight/Request Process Clearly Stated Expectations for Easy-to-Follow Program Processes

  7. Part Four- Preparation • Spend Some Time Before Year Starts Talking About Potential Problems • Are files in different places? • Is there a really busy partner with a role? • Think of the one thing that keeps you up at night

  8. Part Five- Comprehensive Timeline Each Program Year Task from Beginning to End Who is Responsible? Who will help? Due Date? Actual Date? If items aren’t finished on time, revise and remind
